Today, Conexio Care serves as one of Delaware's largest nonprofits, collaborating with government, community, corporate, and other philanthropic partners to maximize services for our state's most vulnerable The Program Ambassador is a member of the clinical team who assists clients and guests in gaining access to safe, timely, and quality services.
The Program Manager is the supervisory and administrative leader of the clinical team that directs compassionate care and assistance to persons with intellectual, developmental, and behavioral health challenges who require assistance to achieve their fullest potential. The Program Manager is responsible for directing the daily operations of multiple residences that allow the persons we serve to live independently; ensure their wellness, health, and safety; and provide the managerial oversight necessary to operate within budgetary parameters and comply with all policies, procedures, and regulatory requirements. The Program Manager directs the interdisciplinary teams that provide the necessary services to meet the environmental, physical, psycho-social, and personal needs of the persons in our care. The Program Manager advances the mission of organization by promoting a culture of compassion, hope, service, and collaboration and works effectively with staff to achieve these goals.

Provides supervisory and administrative oversight to ensure the success of IDS residences and programs, including compliance with all regulations, agency policy and procedures, and within budgetary limits.Directs the staff in the provision of program activities that enhance independence, dignity, and social responsibility, based on the individual's person-centered plan.Participates in the hiring, supervising, training, and, when necessary, disciplining of direct care staff.Reviews the time over and time for each assigned residential program and provides justification for variances.Contributes to the development and implementation of training and other staff success activities.Assesses staff competence and develops individualized, person-centered training to improve the delivery of services that promote the safety and well-being of individuals we support.Monitors and ensures program compliance with the policies and certification standards for DDDS/LTC/HCBS as it applies to Community Living Arrangements (CLAs), Neighborhood Homes (NHs) and Day Treatment Service Programs.Oversees, in collaboration with the Director of Quality Assurance, the provision of services (nursing, direct care, behavior analysis, mental health) and consumer response to treatment interventions with a focus on required documentation in the electronic record.Provides quality assurance by regularly reviewing environmental conditions, personal spending records, medication records, Client Oriented Records, and information entered in the electronic record.Assists the Executive Director with evaluating referrals for residential services.Coordinates with the Executive Director and Director of Quality Assurance to establish new development for CLAs and NH.Reviews support needs of individuals served in the IDS program, annually or as requested, to assist with determination of adequate funding provided.Assists the Executive Director in providing the necessary documentation for rate adjustments, request for alternative placements and other financial and programmatic decisions.Communicates effectively with the Executive Director and Director of Quality Assurance and other team members to guarantee consistent and cohesive delivery of IDS services.Participates in providing after hours response to address emergent issues within the IDS program.Creates and maintains an environment which fosters team decision making and treatment planning whereby each member is held accountable for team decisions.Attends agency-wide Performance Improvement and Clinical Care Committee meetings as requested or necessary.Adheres to all laws, regulations and internal policies regarding abuse, neglect and mistreatment, management of consumer funds, ethical behavior, confidentiality, and privacy.Assists in maintaining the well-being and safety of persons servedRequired Education and Qualifications:
Bachelor's degree in rehabilitation, mental health, or human services field preferred.5 years of experience working with adults with intellectual, developmental, cognitive, or psychiatric disabilities2 years of experience in leadership or supervisory positionPrevious experience supporting people who struggle with intellectual, developmental, or behavioral health challenges preferred.Additional Qualifications:
Computer knowledge of MS Office, and other HR applications is a plus, not required.Possess strong verbal and written communication skills.Must be proactive, organized, and possess great follow-up skills.Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.3 years of motor vehicle record and a valid DE drivers licenseProblem solving skills
