Bilingual in English/Spanish Required - Additional $1.00 per Hour (Must Pass Agency Bilingual Test)The Case Manager III, Housing Navigation (Bilingual) is a direct service position that provides housing-focused services to transition individuals and/or families from experiencing homelessness to housing. The Case Manager III, Housing Navigation (Bilingual) completes participant-centered assessments, creates individualized case plans, and facilitates mutual goal setting with the purpose of navigating housing resources/services to secure housing. Housing navigation activities include determination and referral to appropriate programs, assessment for rental subsidy assistance, facilitation of support services, and resource/service education. Target populations are identified and referred to navigators through Coordinated Entry, as well as from internal programs and outside agencies. The Case Manager III, Housing Navigation (Bilingual) often works outside of the office to meet program participants at an assigned program site. The Case Manager III, Housing Navigation (Bilingual) works with the internal network and external partners, maintains professional relationships and communicates effectively with all internal and external partners using shared tools provided by the agency. The Case Manager III, Housing Navigation (Bilingual)’s knowledge and understanding of relevant resources is housing-focused, trauma-informed, and oriented toward individual/family resilience. The Case Manager III, Housing Navigation (Bilingual) provides supervision and support to the Case Manager I, Housing Navigation and Case Manager II, Housing Navigation. The Case Manager III, Housing Navigation (Bilingual) supports program data collection and reporting.Essential FunctionsConduct screening and assessment with participants with the objective of determining housing barriers, and appropriate housing intervention.Divert, link, or schedule individuals and families with the appropriate housing-focused resources and/or services.Practice assertive engagement with individuals and families through persistent and creative activities to motivate, encourage participation, and promote follow-through.Provide team support to supervisor, shelter staff, and participant navigators to support participant’s success.Maintain up-to-date and thorough Case Records and document service encounters as required, input data in a timely manner in Homeless Management Information System (HMIS), DataLink, and/or Smartsheets.Respond to calls, emails, update requests and other inquiries in a professional and timely manner.Follow all best practices standards of care and Catholic Charities’ Policies and Procedures, including Release of Information, informed consent, confidentiality, and file documentation.Complete required trainings and certifications within the established time period.Complete more than one training and/or certification to be able to provide specialized services.Serve on an agency or community service committee representing the department and/or organization.Conduct case management related duties independently, exercise sound professional and case management judgement, maintain Case Records with minimal documentation errors and/or corrective actions.Supervise a team of Case Manager I, Housing Navigation, Case Manager II, Housing Navigation and Program Support staff members.Prepare monthly program data reports and client summaries as assigned by the Program Manager, Housing Navigation.Support staff members in tracking and sharing information for reporting and contract compliance.Participate in program improvement/development and cultivate partnerships with contractors or other providers.Other ResponsibilitiesWork with Diocese of Santa Rosa and local parishes as appropriate and requested.Perform other related duties as assigned.Agency CultureIt is essential that all employees of Catholic Charities aspire to the following:? A commitment to innovation and to what is possible.Education, Experience, and Skills RequiredAssociates Degree in a social service or related field, bachelor’s degree preferred. One to two-years’ experience as a case manager in housing navigation or equal work experience or a bachelor’s degree, one year of experience supervising staff and program operations with an ability to implement systems required.Bilingual (English/Spanish), passing score on the Catholic Charities’ test of oral and written Spanish fluency required.Understanding of causes of homelessness, experience working within social services and familiarity with community resources. Knowledge of Housing First principles and methods preferred.Ability to work under pressure with a positive, solution-oriented approach.Computer literacy required, including experience with Microsoft 365 Suite including Excel.Strong written and verbal communication; organization, conflict resolution and problem-solving skills.Excellent interpersonal skills, cooperative, friendly, proactive, and helpful attitude with clients and co-workers. Ability to work closely with clients and other employees to ensure a positive and constructive environment within the program or department and throughout the Agency.Passion and enthusiasm for the mission of Catholic Charities and its clients.Valid driver license and at least state required minimum of automobile insurance, clear DMV driving record and ability to provide own transportation.
