OverviewBilingual in English/Spanish Required - Additional $1.00 per Hour (Must Pass Agency Bilingual Test)The Case Manager II, Housing Stabilization (Bilingual) is a direct service position that provides housing-focused services to transition individuals and/or families experiencing homelessness to housing. Responsibilities include completing participant-centered assessments, creating individualized case plans, and facilitating mutual goal setting to stabilize housing resources/services. Housing Stabilization activities include determination and referral to appropriate programs, assessment for rental subsidy assistance, facilitation of support services, and resource/service education. Target populations are identified and referred to stabilizers through Coordinated Entry, as well as from internal programs and outside agencies. This position often requires working outside of the office to meet program participants at assigned program sites or in homes. The Case Manager II, Housing Stabilization (Bilingual) collaborates with internal networks and external partners, maintaining professional relationships and effective communication using shared tools provided by the agency.Essential FunctionsConduct screening and assessment with participants to determine housing barriers and appropriate interventions.Divert, link, or schedule individuals and families with appropriate housing-focused resources and/or services.Practice assertive engagement with individuals and families to motivate and encourage participation.Provide team support to supervisor, shelter staff, and participant navigators to support participant success.Maintain up-to-date and thorough case records and document service encounters as required.Respond to inquiries in a professional and timely manner.Follow best practices and Catholic Charities’ Policies and Procedures.Complete required trainings and certifications within the established time period.Serve on an agency or community service committee representing the department.Conduct case management duties independently, exercising sound professional judgement.Other ResponsibilitiesSome travel within Sonoma, Lake, Mendocino, and Napa counties as needed.Work with the Diocese of Santa Rosa and local parishes as appropriate.Perform other related duties as assigned.Agency CultureIt is essential that all employees of Catholic Charities aspire to the following:- A commitment to the agency’s mission, vision, and values.- A commitment to excellence in everything we do.- A commitment to performance and quality improvement.- A commitment to outcomes and measured results.- A commitment to innovation and to what is possible.Education, Experience, and Skills RequiredAssociates degree in a social service or related field required; bachelor’s degree preferred.One to two years’ experience working as a case manager in housing stabilization or equivalent experience required.Bilingual (English/Spanish), passing score on the Catholic Charities' test of oral and written Spanish fluency required.Understanding of causes of homelessness and familiarity with community resources.Ability to work under pressure with a positive, solution-oriented approach.Computer literacy required including experience with Microsoft 365 Suite.Strong written and verbal communication; organization, conflict resolution, and problem-solving skills.Excellent interpersonal skills, cooperative, friendly, proactive, and helpful attitude.Passion and enthusiasm for the mission of Catholic Charities and its clients.Valid driver license and at least state-required minimum of automobile insurance.Must pass DOJ background clearance (fingerprinting) prior to start of employment.Equal Opportunity EmployerCatholic Charities of the Diocese of Santa Rosa is an Equal Opportunity Employer, seeking the best and brightest.
