GPH is seeking a dynamic, flexible social worker for its Healthcare for the Homeless (HCH) sites. This social work position presents a unique opportunity to support a wide range of individuals currently or recently experiencing homelessness. The HCH social worker works between two locations, the Bayside clinic and the Riverside clinic at the Portland Homeless Services Center, with the potential opportunity for street outreach. The ideal candidate will have a non-judgmental, person-centered approach, familiarity with harm-reduction, and a commitment to promoting the dignity and respect of all peoples. The HCH social worker consistently collaborates with medical and psychiatric providers as part of the GPH integrated care model, as well as with external community partners and case managers. The HCH sites are fast-paced, nontraditional outpatient settings that serve a wide range of individuals with diverse, complex needs. The HCH social worker must be comfortable working with patients in active substance/polysubstance use, patients who require the use of an interpreter, and patients who are unhoused and/or unsheltered.

Master's degree in social work and licensed as a Clinical Social Worker by the State of Maine required.Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.Competence in emergency assessment, planning, intervention, and evaluation of various psychiatric/chemical dependencies to adequately meet position requirements.Generous Benefits Package:
