General Summary:
The staff Physical Therapist has the primary responsibility for direct patient care activities. This position is a Delaware licensed Physical Therapist professional who is capable of evaluating patients and delivering appropriate individualized treatment based on patient assessment/needs and medical condition. The Physical Therapist must be able to provide services across the lifespan and work in both outpatient and inpatient settings as appropriate to the employee's skill set and department requirements.
Responsibilities:
1. Provision of Care/Services: a. Provides comprehensive evaluations and treatments to patients across the lifespan as appropriate. b. Modifies treatment and plan of care as indicated; notifies medical team when indicated. c. Incorporates a variety of treatment techniques based upon the needs of the patient. d. Appropriately prioritizes daily activities with consideration of individual and departmental needs. e. Focuses on achieving the greatest well-being and the highest potential for the patient/client. f. Collaborates with nursing and allied health staff to assure optimal patient care. g. Demonstrates competence on departmental skills checklists. 2. Documentation/Data Management: a. Documentation is timely, accurate and completed by end of work shift daily, manual and electronic. b. Documentation reflects performance of procedures and treatments in the plan of care and physician prescription. c. Assures documentation in the patient record is sufficient for other staff to be able to carry out a total and appropriate plan of care. d. Compliance with legal, departmental, organizational, accrediting agency policies including protocols and multidisciplinary tools. e. Bill patients accurately. Billing is complete on the same day as treatment rendered. f. Accurate and time data collection per department requirements. 3. Communication: a. Maintains ongoing and effective collaboration between front office and other therapy staff. b. Provides Supervisor/Manager with ongoing feedback of patient and/or departmental concerns c. Facilitates internal and external communication by participating in collaborative relationships with other health practitioners and the public at large 4. Fiscal Responsibilities: a. Behaviors demonstrate a positive commitment to departmental fiscal accountabilities and goals b. Complies with established departmental productivity standards 5. Department Participation: a. Takes responsibility for content presented at all staff meetings and applies information to daily activities as directed b. Regularly attends, actively participates and contributes to departmental in-services/meetings c. Participates in appropriate educational programs offered by the organization d. May attend continuing education courses that benefit the organizational objectives as directed e. May serve on departmental/organizational committee as approved by Manager 6. Oversight: a. Appropriately utilizes, communicates with and oversees the activities of support staff, students and volunteers as directed by the leadership team and within the licensure regulations in the State of Delaware b. Completes, in a timely fashion, all necessary paperwork related to student assessment as required by affiliating school 7. Performance Improvement: a. Accurately identifies, understands, and participates in Performance Improvement activities within the department, as directed by Supervisor/Manager b. Demonstrates knowledge and understanding of current organizational Performance Improvement initiatives c. Communicates with the Supervisor/Manager to utilize current Performance Improvement data to improve clinical outcomes and efforts to achieve "best practice" standards of care 8. All other duties as assigned within the scope and range of job responsibilities.
Required Education, Credential(s) and Experience:
Education: Bachelor Degree ; Physical Therapy ; Credential(s): Physical Therapist Basic Life Support ; Experience: Preferred: One year of experience in inpatient or outpatient setting. Preferred Education, Credential(s) and Experience:
