Position Summary
Summary:The Care Manager is accountable for the management and coordination of the patient's plan of care beginning with the admission, active phase of care, and addressing of discharged needs and concerns to ensure the patient receives appropriate, cost effective, quality care during their in-patient stay.
Qualifications
Responsibilities:Education Required• Must complete in-house orientation program• Graduate of an accredited Registered Nurse diploma or college program.• Bachelors Degree in Nursing preferred.
Experience And Skills• Excellent interpersonal skills• Good oral and written communications skills.• Speaks, reads and writes English• At least 3-5 years clinical experience as a registered nurse in an acute care hospital required• 2 or more year's hospital case management, utilization reviews or discharge planning experience preferred.• Recent acute care hospital experience preferred• 1-2 years of critical care experience preferred
License/Certifications• Licensed and currently registered or otherwise duly authorized to practice as a professional nurse in New York State.• PRI certifications.• Case Management certification strongly encouraged.

Other information:Working Conditions• Personal Protective Equipment (PPE) worn• Extensive computer time.• Walking the units.
