About the Position

The expected salary role for this position upon hire is $74,390- $110,464.

WHO WE ARE LOOKING FOR

The Medicaid Long-Term Care Fund Services Program, in Aging and Disability Services, is currently recruiting for an enthusiastic professional Community Health Nurse II who is dedicated to serve seniors, Adults, and children at risk and/or with disabilities in a multidisciplinary setting.

What You'll Be Doing

Community Health Nurse II who is dedicated to serve seniors, Adults, and children at risk and/or with disabilities in a multidisciplinary setting. Employee provides assessment and clinical intervention to medically fragile, disabled, and vulnerable clients (pediatric and adult) in the community.

Duties also include complete assessments for Community First Choice, Community Personal Assistance Services, Community Options and Medical Adult Day Care Waiver.The nurse is a member of a multidisciplinary Adult Protective Services/Social Services to Adults team that provides professional assessment and clinical intervention to clients at risk of institutionalization and abuse.This employee would also do assessments on clients in the community, who are at risk of nursing home placement, PASRR assessments on patients with MI/ID prior to admission to a Nursing Facility (NF) or seeking to continue to receive service in a NF.GES assessments on older individuals in Emergency Rooms at risk of psychiatric placement in a state facility.The nurse is responsible for making clinical decisions to determine health needs and make recommendations for medical and/or nursing care and follow up.Duties include being on call for investigations of abuse, neglect, self-neglect, and exploitation of vulnerable adults who are at risk in the community and taking steps to ensure their safety in the least restrictive setting.Additional duties include crisis intervention and assisting with documentation and testifying in cases that are taken to the court system.Consultation and collaboration with other staff within the department as well as with collateral agencies such as the MCPD, State's Attorney's Office, County Attorney's Office, private attorneys, and community public and private agencies is required.This is a Community Health Nurse II position that requires a licensed nurse to provide medical assessment, clinical intervention, forensic testimony, and case consultation.This position supports the mandated programs of Adult Protective Services, Preadmission Screening and Resident Review (PASRR) and the Social Services to Adults and Community based Waiver programs.The CHN II provides medical expertise and consultation for both program line-staff and families in the community. The CHN II nurse is responsible for making clinical decisions to determine vital health needs and making recommendations for medical and/or nursing care and follow up.Duties include responding to urgent situations to assess vulnerable adults at risk in the community.The nurse may provide lifesaving intervention, prevent, or facilitate hospitalization, promote, or improve quality of life and prevent serious injury, abuse, neglect, or exploitation. This position requires adult and pediatric assessments for all eligible clients in Montgomery County.Minimum Qualifications

Experience:

Two (2) years of full-time equivalent post-licensure experience as a RN Nurse, one year of which must have been in the nursing area working with seniors, Adults, and children at risk and/or with disability in a multidisciplinary setting.

Education:

Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland Board of Examiners with a Bachelor's Degree in Nursing.

Equivalency:

An equivalent combination of education and experience may be substituted.

LICENSE:• Possession of a current license to practice as a Registered Nurse in the Maryland State, of Maryland issued by the Maryland Board of Examiners is required at the time of appointment. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment.• The State of Maryland accepts as valid the current RN licensure from a candidate's primary state of residence if that state has an interstate compact with Maryland. The states having this interstate compact are Arizona, Arkansas, Colorado, Delaware, Idaho, Iowa, Kentucky, Maine, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, and Wisconsin. (This equivalency only applies to candidates whose primary residence is not Maryland). Any Registered Nurse (RN) or Licensed Practical Nurse (LPN) applying for licensure and living in a member state of the Multistate Licensure Compact must take the NCLEX exam in their home state.

Experience working with seniors, Adults, and children at risk and/or with disabilities in a multidisciplinary setting.Experience working in Nursing Facilities and/ or assisted living facilities.Experience working in a home care setting and/or providing interventions in the client's home.Experience functioning independently in a community setting with clients of all ages.Experience working with clients with Intellectual Disabilities and/or Serious Mental Illness.
