Palliative Care Physician-
Location: Remote- must reside in the state of Indiana.
Responsible for providing oversight and supervision for Advance Practice Providers (APP's) and for providing some direct Palliative Care services to patients.
How you will make an impact:• Provides supervision and collaborative oversight for Advance Practice Providers (Nurse Practitioners and/or Physician Assistants) providing in-home Palliative Care.• Models and Provides direct Palliative patient care by interviewing, examining, and treating clinical patients.• Educates and provides ongoing mentorship to APP's in Palliative Care medicine.• Teaches APP's and provides education to patients and families on disease progression, symptoms, and advanced care planning. Communicates with other providers on the patient's care team and assists in coordinating care.• When appropriate, counsel patients and families on transition to Hospice services.• Participates in quality improvement, management, continuing education, and patient care programs. Assists in the resolution of complaints, requests, and inquiries from patients.
Qualifications:• Requires a MD or DO. Requires current unrestricted medical licensure in applicable state(s) and board eligibility or board certification (Palliative and Hospice Medicine). DEA license required.
