Reporting to the PACE Center Manager, the Occupational Therapist (OT) PRN evaluates PACE participant's level of function to establish treatment programs to improve the level of function or prevent disability by helping participants to maintain and/or restore physical potential by means of such modalities as training, exercise, orthotics, and assistive devices. Plans and provides educational services for other PACE Partners health care personnel. Conducts initial assessments and periodic reassessments and documents and reports changes in a participant's condition consistent with the organization's documentation policies.
Duties and Responsibilities
:
When instructed, conduct an in-person initial comprehensive occupational therapy assessment on all PACE Partners participants upon enrollment and periodically as required by CMS PACE Partners regulations and PACE Partners protocolsRecommend a treatment plan to and coordinate with supervisor, and when instructed, with the interdisciplinary team in the development of the plan of care for each participantWhen instructed, participate as a member of the interdisciplinary team, maintaining regular attendance at all the interdisciplinary team meetingsCommunicate participant changes; collaborate on care planning decisions and coordination of 24-hour careActively participates in the Intake and Assessment care plan meetingsEvaluate participants' needsFollow-up with participants during treatmentWhen instructed, discuss condition and treatment plans, and recommend solutions to participant care problems with the Interdisciplinary Team (IDT)Plan individual and group activities and implement treatment according to planAssemble appropriate equipment and data to structure an environment that will enhance participants' responses to treatmentAssess the home environment for safety and need for equipment and adaptationsAssists with the implementation and coordination of a home modification program, including installation of equipment and other changes to support independence and safetyRe-evaluate participant's level of function to objectively determine participant's response to treatmentAlter treatment plan and activities accordinglyEducate and instruct participants and family members in proper care and use of various supports and exercise programs and in use of durable medical equipment in the homeDocument progress notes for each day of treatment including notations of the participant's status regardless of whether significant changes have occurredDocument participant/family educationExhibit communication skills necessary to interpret age-specific responses to treatmentKnow and adhere to the philosophy and goals of PACE PartnersKeep current and accurate records of evaluations, treatment data, and participant's response to treatment, verbal and writtenParticipate in educational programs and attend all mandatory educational programs.Maintain functional operating level of equipmentInform supervisor of needs for equipment and materialsPerform assigned duties as instructed for all emergency drills or situationsOrient, trains and demonstrates techniques for staff and volunteers individually and in groups as directedAct only within the scope of authority to practiceOther duties as assignedQualifications and Requirements:
Education/Training/Certifications:
Current Florida Occupational Therapy licenseCertification with the National Board for Certification in Occupational Therapy (NBCOT)CPR with First Aid or BLS certification requiredCurrent Florida driver's license with proof of active auto insuranceExperience:
Minimum of 2-years working as an Occupational TherapistMinimum of 1-year working with a frail or elderly populationSkills and Knowledge:
Ability to evaluate, plan and implement a program of purposeful activities to develop or maintain functional skills necessary to achieve the maximal physical or mental functioning, or both, of the individual in his daily occupational performance and the promotion of wellnessDemonstrates clinical skills reflecting knowledge of resources and basic treatment skills in the following areas activities of daily evaluation, adaptive equipment, orthotics design, and application, upper motor neuron disease, rheumatology, sensory-motor integration disorders, and the adaptation of physical environments for the handicappedAbility to interpret age-specific data and provide age specific treatmentSupervisory, leadership, and management skillsGood judgment, problem-solving and cognitive skills.Knowledge of gerontology and enthusiasm for treating the geriatric populationPatience and strong interpersonal skillsKnowledge of common safety hazards in the home and improvements to establish a safer environment
