Overview
St. Luke’s Health system is looking for a Nurse Practitioner or Physician Assistant to build our Hospital at Home program. This innovative care model delivers hospital-level services directly to patients' homes. By extending acute-level care to patients in their homes, Hospital-at-Home programs have demonstrated significant cost savings, improved patient outcomes, and elevated patient satisfaction. Collaborating closely with operational partners, the Advanced practice providers will be supported physicians, focusing on clinical outcomes, program evolution, operational efficiency, and inter-provider collaboration. This position will help support inpatient, virtual and at home patient visits.
What to Expect:
Schedule will consist of 7am-7pm, with potential for expanded hours in the future.
Highly competitive base salary with potential for production and quality incentives;
Hospital employed with great benefits, retirement, malpractice, vacation and CME allowance;
Qualifications:
Education: Graduate education from an accredited Nurse Practitioner or Physician Assistant program or qualifying education as designated by the Idaho Board of Nursing or Medicine.
Experience: previous Home Health or Hospitalist Advanced Practice Provider experience strongly preferred
Licenses/Certifications: License and certification, as required by the specialty, or be eligible to obtain licensure from Idaho Board of Nursing or Medicine to practice as a Nurse Practitioner or Physician Assistant. Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.
