SummaryUnder the direction of the Program Manager, the Driver will assist with the delivery of direct services via safeto and from program activities. Transport clients between housing facilities, social service agencies, potential housing units and program sites.
Essential Functions
The task, duties and responsibilities of the job are included, but not limited to the following.Transport all scheduled appointments.Maintain records of mileage and destinations of all client transport.Maintain documentation related to the gas card.Communicate and coordinate with program staff to schedule and map out client transportation needs.Transport Inter-office mail.Transport materials between program sites.Maintain records on the maintenance of the transport vehicle such as: tune-up, breaks, oil change, car wash and cleanliness of the van.Keep all proper and updated documents in the vehicle.Transport clients between facilities, social service agencies, potential housing units, and program sites.Assist with rental assistance payment deliveries to landlord/property managers and drop off check requests to SSG fiscal needed.Operate company vans and larger vehicles in accordance with the federal and state driving guidelines and safety codes.Adhere to Division policies and confidentiality, treat clients, co-workers and partners with dignity and respect.Enter case note for any transportation service that was provided to program participants within 24 hours.Represent the agency in a professional manner at meetings and community events.Enter Case Notes in DAP (Data, Assessment, Plan) form. Additionally, case notes will be detailed and grammatically correct.Submit Productivity Reports weekly at the close of business to direct leads/supervisor on Fridays.Attending weekly conference meetings or as needed.Provide the Supervisor/Program Manager with all the required outcomes as requested.Maintain appropriate boundaries and adhere to SSG's Code of Ethics and HOPICS Core Values.Regular attendance required.Perform other duties as assigned by the Supervisor, Program Manager, Division Director, and Deputy Director.Maintain files/records on client's services in compliance with HIPPA, 42 CFR Part 2 and other funding requirements for audit purposes.Timely completion of required reports and documentation.Attend mandatory meetings and training courses.
Minimum Qualifications - Knowledge, Skills and Abilities Required
Knowledge, Skills, Abilities Requires: High School Diploma or GED equivalent. Submission of an H-6 printout from the California Department of Motor Vehicles. Valid Driver's License and Proof of Insurance. Ability to communicate effectively with minimal supervision, as well as being a team member. Verification of Employment Eligibility and Background Clearance. TB test is required every year or as needed. CPR and First Aid Certification are required every two years or as needed.
Supervisory Responsibilities
This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)
The Driver will work in a field setting and spend most of his or her day on the road, behind the wheel, driving on streets and freeways. The Driver may be exposed to aggressive, sometimes noisy, loud and disruptive clients, and must work in conjunction with other staff members, interface with courts, LAPD, LA County Probation, Sheriff's Department and Mental Health Departments and other community agencies. Evenings and weekends are required. The Driver will some responsibility to work in noisy environments where adults are free to talk loud and expressive. The Driver will be working in encampments in South Los Angeles, dealing with angry individuals with complex mental health, substance use and physical disabilities. There may be dangerous pets at the encampment sites, as well as risk of exposure to infectious disease, vermin, discarded trash (including needles), traffic dangers and chaotic working environments.
Physical Requirements
The Driver typically spends time sitting, standing, typing, writing, walking, driving, carrying (max 50lbs), listening and speaking.
Mental Requirements
This position will require the individual to be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules: be able to process information, think and conceptualize.
