Overview
Eligible for $5,000 Sign On Bonus. This is a hybrid remote role.
Evaluates, assesses, and plans nutrition care to meet patient nutritional needs.
Responsibilities
Assesses patient nutritional status using documented medical information and established nutritional standards.
Counsels patients and/or families by identifying nutrition-related problems. Develops plans of instruction and nutritional care, and monitoring results.
Identifies inpatients at nutritional risk and documents recommendations.
Diagnose and develop intervention for patients with malnutrition
Participates in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
Conducts education programs for patients, community groups, and staff.
Precept and mentor new staff or existing staff in clinical specialty area.
Develop therapeutic relationships with multiple subspecialty care team members and providers that enables planning and participating across a broader spectrum patient care.
Cross-covers a broader spectrum of patient care areas and subspecialties as needed.
Performs other duties as required or assigned.
Qualifications
Bachelor’s Degree from an accredited program in nutritional sciences (prior to 2024).
Master’s Degree from an accredited program in nutritional sciences (2024 and after).
Minimum 2 years’ related experience preferred.
Required Licensure/Certifications
Current NH Dietitian license.
Current Registration by the Commission on Dietetic Registration.
