POSITION SUMMARY

The position serves as member of the UKSM-W Medical Practice Association HIV Program and Midtown clinic staff. The primary function of this position is setting up out-patient appointments, procedures, and ancillary services for HIV positive patients needing assistance outside the IM Clinic. Types of referrals include but are not limited to specialty care, dental, mental health, eye exams etc.)

Position has dual reporting to the Midtown Clinic Manager and the HIV Program Coordinator. Works closely with healthcare providers to identify outside service providers that will perform necessary procedures in a timely and professional manner and at the established rate of compensation. Position is funded through a Ryan White grant and part of the HIV program team.

Essential Functions Statement(s)

Schedules physician appointments, procedures, and tests to be done outside UKSM-W. Schedules appointments with appropriate facility, notify the patient, and instruct on any preparations that may be necessary. Provide directions to location and work with case managers on client transportation assistance if needed.

Coordinate intake information, including the gathering of medical records for newly diagnosed HIV, returning, and new to clinic patients, scheduling first appointments with providers and case management, as needed

Assists with identifying new referral agencies when needed with approval of the Medical Director.

Documents in patients EMR all progress taken related to individual referrals

Handles physician and patient questions about referrals, admissions, and testing.

Returns patient calls regarding referrals and appointment scheduling.

Identifies funding sources for referral procedures to assure that Ryan White grants are the payer of last resort

Attends Ryan White Program team meetings

Works in conjunction with Ryan White staff to verify eligibility of patient to receive services, and what procedures or services are allowable expenses (HRSAs policy #16-02)

Communicates concerns regarding issues impacting patients care to providers, case managers or other individuals as appropriate

Informs designated clinic or HIV program staff of missed appointments, disruptive behavior or other incidents impacting patient care on the part of the patient or service provider or their representative.

Maintains and further develops the Excel system of tracking patients status in the referral process

Addresses insurance issues including prior authorizations, and pre-certifications

Manages referral progress from beginning through completion. Follows up to verify patient attends appointments, procedures were performed as anticipated, and necessary reporting from referral agency is received by IM provider.

Maintains knowledge of, and adheres to, all standards for Universal Precautions Operating Procedures.

Follows HIPAA guidelines regarding patient health information (PHI)

Education: High School Graduate or General Education Degree (GED)

Experience: 2 years minimum in medical related field required, prior experience with EMR systems and knowledge of medical terminology preferred.

SKILLS & ABILITIES

Computer Skills

Ability to learn and work with electronic medical records scheduling and documentation. Basic Microsoft office familiarity.

Confidentiality of all data, including PHI, billing information. High interpersonal skills including demonstrated ability to relate well with others in the execution of duties and the ability to communicate effectively. Individual must have excellent organizational skills, the ability to handle multiple priorities and meet deadlines. Individual must be a team player, but also be able to function independently. Must be able to work with a diverse population.

Additional duties as required.. Maintain proficient knowledge of organizational emergency procedures.

Salary: \$16.50 per hour
