Salary posted is for
fully licensed counselor
and is based upon experience.
Limited Permit salary: $58,617
Assertive Community Treatment (ACT) is a recovery-oriented practice that provides evidence-based treatment and support services delivered by a mobile, multidisciplinary behavioral health treatment team. The services provided by ACT include a full range of clinical treatment. Such care includes, psychosocial rehabilitation, care coordination, and community support services. These services are designed to promote recovery by improving psychiatric symptoms, preventing relapse, reducing substance use, teaching skills, providing direct assistance, and securing community resources necessary for successful overall functioning in work, school, community, home, and social relationships.
Responsibilities:
Working as part of a multi-disciplinary mobile treatment team
Provide home visits, community visits, and in office appointments
Provide mental health support
Complete service planning and assessments
Provide care coordination and clinical case management services as needed
Coordinate with internal and external providers
Assessment of client mental health and/or substance abuse diagnosis
Knowledge of principles and practices of harm reduction
Promote a culture of inclusion and belonging
Requirements:
LCSW, LMSW, LMHC (supervision provided) and 2-4 years prior experience providing therapeutic intervention. Must be comfortable facilitating groups, and completing community visits/home visits. Must have reliable transportation and a Clean/Valid NYS Drivers License.
