Provides intake, case management, and care coordination services. Makes referrals to other agencies and health professionals.Essential Job DutiesAssesses client needs and independently, or in consultation with a social worker, physician or other licensed professional develops a treatment plan to prevent HIV infection.Communicates verbally and in writing with medical providers, clients and other human services professionals.Maintains detailed records of cases and assembles these into periodic reports.Provides education to the public regarding HIV and Hepatitis C prevention strategies.Assists walk-in clients with rapid HIV and Hepatitis C testing.Organizes and participates in outreach and testing events.Reviews and updates program performance measures and strategic plans.Examples of Other DutiesPerforms other related duties as assigned.NOTE:

Each position may not be required to perform all of the essential job functions listed.Required Education and ExperienceGraduation from an accredited college or university with a bachelor's degree in a social or behavioral science.Experience as described in essential duties may be substituted on a year-for-year basis up to a maximum of four years for the required education.Preferred QualificationsKnowledge related to HIV prevention and risk reduction strategies.Knowledge, Skills and AbilitiesStrong computer skills with proficiency in Excel, Word, SharePoint, PowerPoint, WebEx, and Outlook.Strong verbal and written communication skills.Knowledge of interviewing techniques.Ability to obtain information from medical reports and compile information into case records.Ability to establish and maintain effective working relationships with others.Ability to maintain detailed and accurate records and to prepare summary reports.Licenses and CertificatesSome positions within this classification may require possession of a valid driver's license equivalent to a non-commercial, class C Maryland driver's license at the time of appointment.Proof of Degree EquivalencyApplicants who have obtained a degree from outside the United States of America are required to submit degree equivalence documentation from World Education Services, Inc.
