A Human Services Associate vacancy exists in the Department of Aging as a Maryland Access Point Specialist. The office for the current position is located in Parkville, Maryland.

Proof of licenses, certifications, and/or education must be submitted with each application.Job PurposeUnder general supervision in the Home and Community Based Services (Waiver) program:Serve as a Supports Planner within the BCDA (Baltimore County Department of Aging) AAA Supports Planning Agency (SPA).Provide services to older adults, and individuals with disabilities who meet the State of Maryland Department of health’s medical, financial and technical criteria.Utilize the Maryland Long Term Services & Supports (LTSS) system to manage cases.Document and bill for services rendered in the Supports Planning services rendered in LTSS.Maintain expertise in the field of Home and Community Based Services for the following programs:

Community Personal Assistance Services (CPAS).Community First Choice (CFC)Increased Community Services (ICS)Community Options Waiver (COW)

Render assistance with program enrollment processes and redeterminations.Conduct in-person client visits in community settings which include their home, Assisted Living Facility, or Medical Adult Day programs throughout Baltimore County. On occasions, visits may also take place in a nursing home setting.Adhere to COMAR regulations, and guidance from Maryland Department of Health (MDH) related to rendering Supports Planning services.Develop a person-centered plan that encompasses program and community services.Conduct monthly calls and quarterly visits to assess participant’s need, supports and service delivery.Participate in multidisciplinary team meetings to support the coordination.Accurately input daily billing/documentation in the MD LTSS tracking system.Coordinate with local health departments, Maryland Department of Health, community and program providers, and other stakeholders to ensure the delivery of quality and person-centered services.Train participants and providers on program eligibility and services, self-direction and technology systems.Maintain accurate up-to-date documentation for each participant served.Identify, document, and communicate to appropriate authorities, all reportable events to include abuse, neglect and/or exploitation.Facilitates the transition of participants from the nursing facility out into the community.Follows all COMAR regulations, State of Maryland program guidance and company policies.Examples of Other DutiesPerforms other duties as assigned.NOTE:

Applicants must state the dates and duties of past and present experience clearly and completely for evaluation purposes.Employees hired after July 1, 2022 are required to participate in the Baltimore County Employees’ Retirement System, with very limited exceptions.This position requires satisfactory completion of County sponsored CPR and First Aid training.This position must maintain licenses and certifications throughout employment.Physical and Environmental ConditionsThese positions are essentially sedentary with occasional walking, lifting, or other restricted physical activities.
