The Old Colony Y Community Based Program is currently seeking a part time Occupational Therapist who is energetic, flexible in availability to include after school and weekend work hours, and has a passion to make a difference for children and their families in Southeastern MA communities.! The Occupational Therapist will provide services in our home based therapeutic care program to individual youth experiencing mental health and occupational health needs, and their families, referred by the Department of Mental Health. The Occupational Therapist will work under the supervision and guidance of the Program Director, and will work independently and collaboratively as part of the home based team. The role provides occupational therapy evaluations and re-evaluations, including assessment of prior and current level of function, setting individualized goals, providing direct OT treatments in the home, office, and various community settings, including child and family education, progression of treatment plan and implementation of appropriate interventions, and discharge recommendations according to standards of practice. The OT role will also include reporting on child's progress through appropriate written documentation, including evaluation forms, progress notes, and education forms, and will provide consultation and collaboration with children, parent/caretakers, inter agency and outer agency physicians, caseworkers, clinicians, and other members of the child's multi-disciplinary health care team.

*Graduate of an accredited Occupational Therapy professional program and initial certification by National Board of Certification of Occupational Therapy.

*Current licensing for MA by the American Occupational Therapy Certification Board.

*Registered with the State Board of Registration in Allied Health Professionals.

*Two years experience as an occupational therapist with adolescents or children, preferably in school system or other pediatric practice setting.

*Valid driver's license, clean driving record, and legally registered and insured vehicle for work use. Mileage reimbursement is provided.

*Computer and data entry skills to maintain up to date case record documents on an electronic health record system, and in accordance with state and local guidelines.

*Effective oral and written communication skills to deliver in-person and telephone contacts, emails, etc. to youth, families, collaterals, and peers.

*Able to sit, walk, stand, bend, and walk up and down stairs as needed during the work day.
