Job Description:
Key Responsibilities:
Patient Assessment: Conduct thorough evaluations of patients to determine their physical capabilities and rehabilitation needs.
Treatment Planning: Develop individualized treatment plans that address specific patient goals, outlining the type and frequency of interventions.
Progress Documentation: Maintain accurate and detailed documentation of patient assessments, treatment progress, and outcomes.
Plan Modification: Regularly review and adjust treatment plans based on patient progress and feedback, collaborating with healthcare team members as necessary.
Patient Education: Provide comprehensive education to patients and their families regarding treatment protocols, adaptive equipment, and self-management strategies.
Discharge Planning: Assist in the discharge process, ensuring that patients have the necessary resources and support for continued recovery after leaving therapy.
What We’re Looking For:
Licensure: Must hold valid Maine State Physical Therapy licensure. Registration as a Physical Therapist (PT) by the National Physical Therapy Exam (NPTE) or similar certifying body is preferred.
Experience: 3+ years of clinical experience in an outpatient setting. Experience treating diverse conditions, including amputees, lymphedema, and vertigo, is highly valued.
BLS Certification: Basic Life Support (BLS) Healthcare Provider certification preferred.
Skill Set: Strong communication skills, adaptability to a fast-paced environment, and the ability to work collaboratively within a multidisciplinary team.
