Public Health Nurse (Temp)

Pay rate : $56.00 hr

Projected Hours : 8:15AM -5:00 PM /5 days per week

Administers all recommended and/or required vaccines according to the current ACIP schedule.

1. Insures that all immunization records (Insight, GRITS, family record) are reviewed

2. Administers all recommended vaccines according to the current years ACIP schedule

3. Documents VIS publication date of all vaccines administered at every encounter on the immunization

super-bill

4. Certifies that VIS and copy of immunization record is given to each client/parent

5. Administers immunization based on CDC’s recommended technique

6. Administers and reads tuberculin skin test and documents results

7. Performs direct input of all administered vaccines in Insight

8. Performs school health related services such as hearing, vision and dental screenings.

Maintains quality control of immunizations

1. Monitors and records refrigerators and freezer temperature at least twice daily (AM & PM)

2. Takes immediate and appropriate actions if temperatures are outside of range and documents actions

3. Responds to vaccine excursion notification and is responsible by rotation for the evacuation of vaccines

before, during and after hours- including holidays, Saturday and Sunday.

4. Notifies supervisor and clinical coordinator (or director of nursing if supervisors are not available) when

refrigerators/freezer temperatures are outside of acceptable range

5. Assists with daily/monthly vaccine inventory count

6. Accounts for all vaccines in individual refrigerator/freezer

Creates and maintains a high-performance environment

1. Documents in clear, complete, and concise manner according to standard.

2. Participates in community events when assigned

3. Refers clients to nurse supervisor or appropriate staff as needed.

4. Provide appropriate education and information to clients on discharge.

5. Utilize all available sources of information (books and internet) to provide maximum patient care.

Demonstrates a thorough knowledge of and adherence to STD protocols; pregnancy test procedures,

ACIP immunization schedule, and immunization policies and procedures including travel vaccines

1. Ensures consent is provided and general consent for services is signed.

2. Assures that all consent forms are in client’s primary language, is a part of client’s record, and is

updated if there is a change in client’s health status,

3. Assures that adolescents do not require parental consent to receive STD services

4. Assures that required educational topic areas are provided to include birth control methods, emergency

contraception, breast and testicular exam, education on STD risk reduction.

Assures that all clients receive history, physical assessment, and laboratory testing

1. Completes a medical history at the initial comprehensive visit (Refer to required services manual)

2. Completes an initial physical exam including breasts (for BCCP patients), pelvis, and rectum (when

indicated).

3. Provides and stresses the importance of pelvic exam including vulvar evaluation and bimanual exam &

Pap smear as indicated.

4. Provides specific laboratory tests for specific methods if indicated or requested by client to include GC

and Chlamydia, pap smear, anemia testing (for positive pregnancy tests), vaginal wet mount, hepatitis A,

B, & C syphilis serology, urinalysis, pregnancy test and HIV testing.

Provides treatment for a variety of STIs as medically indicated in compliance with current CDC

guidelines

1. When medically indicated screens, and treats for HPV, Chlamydia, trichomonas, genital herpes, syphilis,

gonorrhea, HIV screening only

2. Adheres to protocols for abnormal pap follow up (see required services manual)

3. Provides or refers clients for microscopic exam and selected blood tests

4. Provides pregnancy test and counseling in a non-directive unbiased manner

5. If pregnancy test is positive if appropriate provides counseling on prenatal care and delivery, infant care,

foster care, or adoption, or termination. (refer to required services manual)

6. Encourages clients with positive pregnancy test to receive referral for early prenatal care

7. Adheres to all requirements related to providing services to adolescents. (refer to required services

manual)

8. Identifies women with estrogen-exposed offspring. (refer to required services manual)

9. Informs, offers and provide clients with information and VIS

10. Administers Hepatitis A & B, HPV and other vaccines when indicated

11. Accurately completes immunization superbill

12. Informs client of needed follow up visits to complete immunization series

Performs assessment of client’s pregnancy status

1. Reviews and signs the medical history form that client has completed.

2. Obtain gynecologic, obstetrical, surgical, medical, genetic, and social history, review of systems, and

vaccination history from client

3. Reviews and interprets client’s laboratory results, blood pressure, height, and weight measurements

4. Informs client of pregnancy test laboratory results

5. If pregnancy test is negative informs and encourages participation in health center services including

immunizations and family planning

6. If pregnancy test is positive, informs client of actions needed to promote a healthy outcome

7. Informs and offers client Prenatal Case Management services

8. Assesses client’s psychosocial needs

9. Identifies and documents client’s psychosocial needs on form

10. Enrolls client in Prenatal Case Management by accurately completing the psychosocial assessment

11. Accurately completes the Prenatal Case Management agreement between client and PCM manager

12. Assures client signs and dates her signature and writes time of enrollment

13. Informs and promotes health center services including WIC.

14. Refers client to Social Service Technician to apply for presumptive eligibility Medicaid

15. Documents all actions in client’s chart

Collaborates with laboratory staff and epidemiologist to assure required services are provided and

coordinated in an efficient manner

1. Knowledgeable of the needs of Communicable Disease Specialist to provide partner notification

counseling

2. Knowledgeable of laboratory requirements for specific laboratory tests

3. Knowledgeable of the immunization recommendations for clients receiving STD services

Assures that all clients receive counseling

1. Method counseling

2. STD and HIV counseling

3. Immunization recommendations

4. Travel vaccines and prophylaxis

Participates in the planning and evaluation of services to ensure the goals of the center are met.

1. Reviews and ensures follow up (when indicated) all incoming laboratory results

2. Participates in peer review at specified intervals to assure compliance with acceptable standards

3. Monitors clinic flow and informs supervisor of backlog to assure that services are provided in a timely

efficient manner

4. Participation in the Emergency Response Activities at the health center level as well as at the DeKalb

County Board of Health level as needed. These activities include, by are not limited to public health

disasters, providing care in shelters, with limited notice.

5. Actively participates in community outreach activities before and after hours, also on Saturday and

Sunday as requested.

Trains for clinic services

1. Cross trains in all clinic services and sub-programs (i.e. Adult Health (STD and BCCP), Child Health

(immunizations and health checks) and Travel clinics).
