JOB SUMMARY:
The Occupational Therapist contracted through our organization is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement.
DUTIES & RESPONSIBILITIES:Assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives, and caregivers.Provide services as ordered by physicians in the plan of care.Offer patient, caregiver, and family counseling.Provide patient and caregiver education.Communicate with physicians and other healthcare practitioners related to the current home health plan of care.Evaluate patients' functional status and home environment for hazards or barriers to independent living.Develop treatment programs and establish goals for improved function.Train patients in the use of adaptive equipment, prosthetic and/or orthotic devices.Maintain appropriate clinical records and report any changes in patients' condition to the physician.Participate in quality assessment and performance improvement programs.Identify patient and family/caregiver needs for other home health services and make appropriate referrals.Instruct patients' family/caregiver and other healthcare personnel in the treatment regime.Supervise Certified Occupational Therapy Assistants.Instruct, supervise, and evaluate home health aide care when therapy is the only skilled service.Participate in discharge planning for patients.JOB REQUIREMENTS:
Must be licensed where the HHA is located if licensure is required.Must have graduated from an OT program accredited by ACOTE of AOTA or any successor organization.Must be eligible for or have taken the OT certification examination administered by NBCOT.Two years of clinical experience as an occupational therapist.Demonstrates good verbal and written communication and organization skills.Possesses and maintains current CPR Certification.Must be a licensed driver with an insured automobile in good working order.
