Job Title: Occupational Therapist (OT) Classification: ContractorLocation: VariesRevision Date: 7/17/2024JOB DESCRIPTIONSummary/ObjectiveAs part of a vibrant team, participate in providing client related therapy services, including but not limited to, activities which assist clients in restoring or improving independent function relative to activities of daily living within the scope of practice to SummitWest Care clients.Essential Functions1. Performs initial and ongoing client evaluations, including starts of care, recertification for services, and discharge from agency2. Oversight of care provided by agency COTAs3. Provides Occupational therapy services to client according to physician orders and regulations.4. Ensures physicians are notified in a timely manner of changes in client status and physician's orders are obtained as needed for changes to the plan of care.5. Identifies and obtains/makes appropriate modifications to equipment/environment to meet client needs.6. Provides teaching and instruction to client/caregiver to promote physical and emotional health.7. Provide accurate and timely point of care documentation of visits and/or client communications, to be submitted by 9 am the following day, including:• Starts of Care• Recertifications• Discharge from Agency• Missed Visits• Communication Notes8. Provide regular and timely responses to questions and requests sent by administrative team9. ttend required weekly meetings and contribute to the coordination of services through effective communication with other members of the health care team10. Maintains the caseload and agency productivity standards, in accordance with agency standards and best practices.11. Responds to referrals to Occupational Therapy in a timely manner and documents care according to agency standards12. Maintains clients' self-esteem, privacy, and confidentiality of personal information.13. Assures client rights are protected and that all clients/caregivers are made aware of the client's rights and responsibilities.14. Initiates emergency procedures as appropriate.15. Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.16. Performs other duties within scope of practice, as assigned by leadership17. Other duties as assigned.Utilizes established lines of authority and communications:1. Maintains NBCOT Membership Requirements3. Currently licensed and in good standing in the State of Colorado4. Previous home care experience and Occupational Therapy experience preferred.5. Valid driver's license and auto insurance, ability to drive to and from client homesDress Code:1. Clothing should be clean and free from of tears, rips, and stains2. Closed toed shoes are required in the field3. Clothing displaying potentially offensive images and/or language is not permitted4.
