278094- Occupational Therapist - Grand Junction, CO - 13 Week ContractSigma Healthcare Team is looking for an Occupational Therapist who will work onsite in our client's location.
Shift : Monday - Friday 8 am-5pmSummary/Objective
As part of a vibrant team, participate in providing client related therapy services, including but not limited to, activities which assist clients in restoring or improving independent function relative to activities of daily living within the scope of practice to SummitWest Care clients.Essential Functions :
Performs initial and ongoing client assessments and applies appropriate goals and interventions to resolve identified problems.Provides Occupational therapy services to client according to physician orders and regulations.Assures physicians are notified in a timely manner of changes in client status and physician's orders are obtained as needed for changes to the plan of care.Identifies and obtains/makes appropriate modifications to equipment/environment to meet client needs.Provides teaching and instruction to client/caregiver to promote physical and emotional health.Attends required staff meetings and case conferences.Contributes to the coordination of services through effective and timely communication with other members of the health care team.Maintains the caseload and agency productivity standards.Responds to referrals to Occupational Therapy in a timely manner and documents care according to the agency standards.Maintains clients' self-esteem, privacy, and confidentiality of personal information.Assures client rights are protected and that all clients/caregivers are made aware of the client's rights and responsibilities.Initiates emergency procedures as appropriate.Clinical and administrative documentation adheres to agency, professional, and regulatory guidelines for timeliness and content.Performs other duties within scope of practice, as assigned by Sr. Director of Clinical OperationsMay require extended staff schedules.Other duties as assigned.Requirements:
Graduate of an Accredited School of Occupational TherapyMaintains NBCOT Membership RequirementsCurrently licensed and in good standing in the State of ColoradoPrevious home care experience and Occupational Therapy experience preferred.Valid driver's license and auto insurance, ability to drive to and from client homes
Computer Skills:
Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.)Basic Internet navigation skillsCertificates and Licenses:
Valid certification from the National Board for Certification of Occupational Therapy (NBCOT)Valid Occupational Therapist state license (where applicable)
