Civil Service Title: Occupational Therapist

Minimum Requirements:

EITHER:

(A) Possession of a valid license and currently registered by the New York State Education Department as an Occupational Therapist; OR,

(B) Possession of a Limited Permit to practice Occupational Therapy issued by the New York State Education Department.

Desired Qualifications:

Must possess or meet requirements for Civil Service title of Occupational Therapist;

Licensed and currently registered by the NYS Education Department as an Occupational Therapist;

Experience in working with students with disabilities and understanding of child development preferred;

Certification in CPI and/or TCI preferred.

Position Summary/Responsibilities:

Conducting evaluations, reevaluations and consultations including the use of a variety of tools including standardized measures;

Determining recommendations for services based on individual student needs as it pertains to the learning process and modifying as appropriate;

Developing a treatment plan based upon assessment findings, including measurable goals;

Writing and obtaining referrals for prescription of services signed by a physician, physician’s assistant, or nurse practitioner that are Medicaid compliant;

Delivering evidence based treatment and interventions to students district wide (K-12+) in a variety settings;

Acting as a consultant to provide strategies for classroom teachers;

Completing documentation of all evaluations, reevaluations, daily progress notes and discharge summaries per New York State, District, and Medicaid regulations; and documentation of Quarterly Progress Notes and consultations per District guidelines;

Providing training and monitoring of adaptive assistive devices, equipment, sensory based strategies, modifications as necessary and/or self-help techniques to appropriate support staff;

Participating with department and school staff in the formulation of services ;

Collaborating with supervisors, colleagues, physicians, teams, parents, community members and/or other support staff as appropriate regarding students’ needs;

Attending multidisciplinary team meetings ;

Maintaining daily schedule per district guidelines;

Assisting in requesting orders for supplies and equipment;

Pursuing professional development and life-long learning;

Other duties as assigned.
