Empath Home Health, a member of Empath Health, is seeking a Lymphedema Certified Occupational Therapist (OT). The Home Health Occupational Therapist provides occupational therapy services related to patient's level of function and performance of daily living.The Home Health Lymphedema Certified Occupational Therapist:Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.Performs initial and on going assessments to determine level of functioning and patient's ability to perform activities of daily living, including OASIS assessments at appropriate time points.Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.Requirements:Bachelor Degree in Occupational Therapy.Florida licensure in occupational therapy.Minimum of one (1) year experience as a licensed Occupational Therapist.CLT - Certified Lymphedema Therapist in the respective state.Home health care experience preferred.Suncoast Hospice, Empath Home Health, and Empath Health Pharmacy are proud to be accredited by the Joint Commission showing our commitment to quality.
