• Offer advice and information to staff and referring physicians on medical interventions consistentwith the plan of care and/or palliative care needs.• Consult with attending physicians, palliative care staff and referral sources as requested, so pain andsymptom management for patients is effectively and expeditiously accomplished among teammembers.• Provide follow-up evaluation to patients and families and clinical consultation to hospice or palliativecare staff as needed.• Participate in the review and development of practice protocols proposing interventions that areconsistent with palliative care literature and research.• Responsible for making contact with assigned patient and/or family to arrange a meeting to provideinformation to the prospective patient and/or family and/or referring professional. An assessment ofthe patient's eligibility for palliative care services will be completed and communicated to the patientand family.• Based on patient need, consultation and evaluation of the patient will be completed.• Responsible for informing the primary providers and referral sources the outcome of the NursePractitioner consultations and evaluations.• Responsible for documenting interactions between palliative care staff, referral sources, patients andfamilies.• Maintain regular contact with referral sources as assigned.• Complete, maintain and submit accurate and relevant clinical notes regarding patient's condition andcare. Record pain and symptom management changes and/or outcomes as appropriate.• Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy,and communication standards. The clinician is expected to appropriately document at Point ofService, have timely synchronization of patient data & effectively communicate with the Patient CareTeam.• Participate in program development and evaluation of palliative care programs.• Collaborate with consultants as needed.• Present education and information to members of the IDT and/or medical community as needed.• Participate in the resolution of interpersonal conflict and issues of clinical and ethical concern asneeded.• Participate in research activities or special projects as assigned and needed.• Medication management for patient's enrolled in palliative care consisting of:o Review of medications, appropriateness, coverage with pharmacy and collaboration asappropriate.• Other duties as assigned.
