Nurse Practitioner or Physician Assistant (Full-time or Interim)

We are currently assisting a leading Skilled Nursing Facility in Massachusetts with the recruitment of a passionate and skilled Nurse Practitioner. This facility is renowned for its comprehensive care and dedication to patient well-being.
Essential Job Functions:Participate in the medical evaluation of patients admitted to the Skilled Nursing Facility.Evaluate and document the medical necessity for specific levels of care, facilitating transfers if necessary.Provide advice and information to staff and referring physicians on medical interventions consistent with the medical plan of care.Consult with attending physicians and referral sources to ensure comprehensive and efficient medical care for patients.Communicate with patients and families regarding their medical condition and plan of care.Participate in the review and development of practice protocols within the facility.Inform primary physicians and referral sources of the outcomes of Nurse Practitioner consultations and evaluations.Document interactions between nursing staff, referral sources, patients, and families.Maintain regular contact with assigned referral sources.Complete, maintain, and submit accurate clinical notes regarding patients' conditions and care.Utilize electronic medical records to ensure quality care, accuracy, and effective communication with the Skilled Nursing Facility.Provide education and information to the SNF care team and/or medical community as needed.Participate in resolving interpersonal conflicts and clinical or ethical concerns.Engage in research activities or special projects as assigned.Participate in the call schedule with other members of the call team (compensated separately).Perform other duties as assigned.Qualifications:
Experience:Minimum of two (2) years of professional experience in healthcare.Experience in a skilled nursing facility preferred.Experience in acute care in an institutional setting preferred.
Education and Training:Nurse Practitioner or Physician Assistant licensure is required.
License, Certification & Registration:Possess and maintain current CPR Certification.Current Massachusetts licensure as a Nurse Practitioner or Physician Assistant.Valid driver's license.
Other Requirements:Demonstrates excellent verbal and written communication, and organizational skills.Strong observation and problem-solving skills.Experience working with an Interdisciplinary Team preferred.
Working Conditions:
May be exposed to infections and contagious diseases.Contact with patients under a variety of circumstances.Subject to varying and unpredictable situations.Handles emergency or crisis situations.Travel required.OSHA exposure category: Category I (tasks involve exposure to blood, body fluids, tissues).
