About the University of Illinois ChicagoTheDepartment of Pharmacy Practice at the University of Illinois Chicago Retzky Collegeof Pharmacy, is looking for a Clinical Faculty (Open Rank) / ClinicalPharmacist to join our Telemedicine Diabetes clinic. The idealcandidate will be responsible for providing pharmaceutical care to patients inthe area of diabetes management. The pharmacist will join a team of physiciansand advanced practice nurses to provide care to patients in an IllinoisDepartment of Corrections (IDOC) facility via a telehealth model. Thepharmacist will work from the University of Illinois Hospital & HealthSciences System (UI Health) campus located approximately 2 miles west ofdowntown Chicago. The hospital is a 450-bed academic institution locatedapproximately 2 miles west of downtown Chicago. with 21 on-site outpatientclinics and 11 FQHCs. The hospital and clinics serve an urban patientpopulation.

The clinical pharmacist will be assigned to the IDOC diabetes telemedicineclinic 4 days per week with 1 office day per week. The hours will be Monday -Friday during the day. The clinical pharmacist will work under a hospitalapproved protocol to participate directly in the assessment and management ofpatients with diabetes. The pharmacist will recommend and implement anevidence-based care plan in collaboration with the treating credentialedpractitioner, resident or fellow; initiate, modify, or discontinue drug therapyfor treatment of diabetes to achieve therapeutic goals; and order laboratorytests to assist in the management of diabetes. The candidate will also be responsible fornurse, physician, and pharmacy staff development; coordinating and conductingquality initiative projects and developing collaborative treatment and researchprotocols.Theacademic responsibilities of the clinical faculty member (ClinicalAssistant/Associate/Full Professor) include the provision of didactic lecturesand precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2residents. Scholarly activities, including presentations and publications, arean expectation as well. Academic rank commensurate with experience.

MinimumQualifications:Graduate of an ACPE accredited College of Pharmacy PharmD programCandidate must have completed a clinical residency and/or fellowship or have 2-3 years equivalent experience for practice in a clinical setting and/or education/experiential education.PGY 1 Residency or 2-3 years clinical pharmacy experienceLicensed, or eligible for licensure as a professional pharmacist in Illinois as of start dateCandidate must possess knowledge and skills such that they qualify for a non-tenured clinical faculty position with the Department of Pharmacy Practice at the UIC College of Pharmacy

PreferredQualificationsPGY2 Ambulatory Care residency or 2-3 years of equivalent clinical experience is preferredBoard certification is recommended
