Job Objective:

A brief overview of the position.Works collaboratively as an active member of the inter-professional team to provide therapeutic patient and family centered care in the Hematology/Oncology patient population in the Cancer Care Program. Collaborates in the development, execution and evaluation of the multidisciplinary plan of care. Establishes goals and strategies for meeting the care coordination, discharge, and/or continuing care needs across settings with the patient, family, and/or other care providers.

Licensure/CertificationRequired: California RN licensure; Oncology Certified Nurse within three (3) years of hire; American Heart Association BLSPreferred: N/A

ExperienceRequired: Five (5) years of direct patient care experience in Oncology nursingPreferred: N/A

Essential ResponsibilitiesDemonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.Educates patients and families to their oncology care plans and ongoing needs.Consults physician or medical director on complex medical changes and management.Coordinates family conferences with clinical care team based on clinical changes/milestone in case.Collaborates with the patient, family, significant others and health care providers in providing patient care.Delegates and supervises care in accordance with nurse practice acts and professional nursing role.Initiates multidisciplinary collaboration to positively impact the outcomes of health provided.Initiates steps to support and enhance the patient's responsibility and self-determination in decision-making concerning health, treatment and well-being.Involves the individual patient/family in identification of requirements/perceptions of learning needs and preparation to assume self-care on discharge.Shares knowledge and acts as a professional resource to other members of the health care team.Supports and participates in Unit Based Council initiatives.Contributes to professional growth of peers by providing feedback through peer coaching and evaluation.Strives to be a life-long learner continuing to enroll and complete classes and courses to advance and increase new clinical knowledge.Participates in unit/clinic and/or departmental shared governance structure in order to improve care.Assesses, utilizing age specific data, the patient's physical, psychosocial, and spiritual status in a comprehensive manner.a. Demonstrates accurate assessment skills, appropriate to the age of the patient, based on scientific facts and principles.b. Performs initial assessment and completes required documentation within organizational standards.c. Performs and documents ongoing assessments including observation, physical examination, laboratory/test results, and patient response to procedural intervention.d. Participates in the identification and clarification of patient needs, as evidenced by participation in multidisciplinary care planning, to achieve optimal patient outcomes.e. Reviews and revises plan of care according to change in patient's status.f. Completes patient classification for assigned patients as required.

Incorporates patient teaching/discharge planning to promote optimal patient outcomes.a. Provides, evaluates retention of, and documents education to the patient/family/significant other according to organizational standards.b. Coordinates and documents appropriate discharge planning and referrals to ensure continuity of care after discharge.

Recognizes, accepts responsibility for, and maintains commitment to professional development and clinical competency.a. Attends required professional development sessions and staff meetings. If unable to attend, review minutes or discuss proceedings of meetings with peers or managers.b. Identifies personal knowledge deficits and seeks resources/consultation to ensure safe patient care.c. Utilizes all channels of communication including reading organizational email at least once a week, to obtain knowledge, transmit information, address concerns, and solve problems.d. Embraces change and continuously identifies opportunities for improvement by demonstrating a commitment to utilizing evidence based practice.e.
