ResponsibilitiesWork in conjunction with Sales Advisor to develop a service plan for care.
Coordinate client services across the agency and collaborate with all members of the health care team.
Participate in chart audits and activities.
Complete documentation daily with all outstanding items completed no later than start of next business day.
Maintain a full time schedule in accordance with established agency productivity standards.
Attend meetings with Administrator and department on a regular basis to discuss new admissions, potential discharges and problem solving.
Provide instruction and oversee certified nursing assistant/home health aide and companion staff on a timely basis and in accordance with agency, State and Federal regulations.
Collaborate with the Administrator to complete and deliver 90 day, quarterly and annual performance feedback for evaluations.
Utilize office and work time efficiently and perform in an organized manner.
Use, store and maintain equipment and supplies in a safe, appropriate manner.
Assist in orientation of new associates and students to Private Care , including instructing staff on orientation, skill training, testing, and any other training, e.g., soft skills, in-service workshops or other topics related to providing appropriate care to clients. Evaluate competence of staff after training and using evaluation tools (90-day, competency, shared visit, annual evaluation) established by the agency or People & Culture.
Monitor new clinical staff during the orientation period, including making initial shared visit and documentation oversight.
Complete shared visits with CNA/HHA and Companion staff upon hire and annually to ensure competency with role and responsibilities.
Utilize inter-agency and community resources to assure continuity of care.
Follow HIPAA guidelines and maintain all protected health information (PHI) obtained in the course of work as confidential and, as such, should not be a matter of discussion with co-workers or others outside of the Agency.
Attend mandatory staff meetings, team meetings and in-service programs. Fulfill mandatory e-learning courses by the due date.
Perform all nursing duties in accordance with the Florida Nurse Practice Act and AHCA regulations; comply with VNA policies and procedures.
Maintain and update client records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
Follow VNA safety policies/procedures. Report accidents or unusual incidents immediately to the Administrator and People & Culture.
Qualifications
Active RN license in the State of Florida
Current Basic Life Support for Health Care Provider Certification/CPR
Minimum one year work experience in hospice, home health, geriatrics, or medical/surgical nursing
Current and valid Florida driver’s license
Current automobile insurance per VNA requirement
