OverviewVNACJ CHC, a federally qualified health center in Monmouth County, is launching a new integrated behavioral health program at our Keyport Primary Care Center location to best serve the needs of the bayshore area. Integrated services, in addition to medication assisted treatment (MAT), will include mental health treatment, treatment for substance use disorder (SUD), peer support, navigation assistance, and care management, co-located in a community health center offering primary care, on-site lab, dental care, and other specialties.
We are seeking a Nurse Practitioner who will be responsible for providing holistic primary care services with an emphasis on providing treatment with medications for opioid use disorder (MOUD) as part of a medication assisted treatment (MAT) program.

ResponsibilitiesSupports organization’s mission by striving for excellence in all aspects of their job description with a focus on positive interpersonal relationship with co-workers up, down, and across the organization
Provides direct patient-centered services to primary care patients and certain special populations within established procedures and productivity guidelines
Provides initial evaluation, diagnosis, treatment, and ongoing support for individuals with complex addiction and substance related health conditions
Maintains complete patient records using the Electronic Health Record and complies with all reporting mechanisms and billing/coding guidelines
Ensures activities are in compliance with the New Jersey Department of Health Ambulatory Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations, and insurance carriers
Collaborates in team-based care in the Patient Centered Medical Home model; ability to work cohesively with diverse care teams
Participates in Peer Review and other Quality Improvement activities as requested
Collaborates in the provision of in-service education for staff
Adheres to CHC infection control and safety policies, and guidelines of regulatory agencies
Participates in community outreach programs regarding CHC services and programs

QualificationsJob Competencies & Minimum Qualifications :
Active and unrestricted licensure as an Advanced Practice Nurse in the state of New Jersey
Master’s degree in Nursing from an NLN accredited program
Certified as a Family or Adult Nurse Practitioner with prescriptive authority; maintain CDS and DEA
2+ years of primary or ambulatory care advanced practice nursing experience preferred
1+ year of experience providing addiction medicine services
Must be eligible to participate with all public and private insurance payers that the CHC participates with
Working Conditions/Physical Demand : Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact.
