Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care.
Position Summary :
Community Health Center is seeking a Family Practice Advanced Practice Nurse for their Monmouth County, NJ Facilities.
This position will be responsible for providing primary care services to clients of the VNACJ Community Health Center in Freehold.
ResponsibilitiesPrincipal Responsibilities:
Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
Provides primary care services to clients and families within established guidelines
Monitors quality and appropriateness of services provided
Initiates client teaching for individuals and families
Prescribes medications as needed
Participates as part of the health care team
Participates in community outreach to educate the community regarding CHC services
Adheres to agency infection control and safety policies and guidelines of primary care regulatory agencies
QualificationsJob Competencies & Minimum Qualifications :
Licensure as an Advanced Practice Nurse in NJ
Master’s degree in Nursing from an NLN accredited program
Maintains current CPR and professional certifications
Excellent primary care skills and thorough knowledge of current practice trends
Strong verbal and written communication skills
Excellent interpersonal skills
Working Conditions/Physical Demand : Working indoors.
Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact.
