The
MYPAC
Therapist
provides best practices, abides by MYPAC guidelines as set forth by DMH, quality clinical services, ensures all documentation is completed per Payer’s, state licensing agent’s, COA, & HCA’s standards and established guidelines. must have a minimum of 5 clients and a maximum of 20 clients and be a
fulltime
MYPAC therapist.Communicate with customers, colleagues, and other individuals to answer questions,
disseminate
or explain information, and address service barriers.Develop treatment plans with the input of the child/youth and family and the Clinical Supervisor.
In the event that
the child/youth
in
enrolled in Wraparound Facilitation, the therapist will actively
participate
in the development of the ISP in the first family team meeting and will continue meetings
on a monthly basis
.Provide individual, group, and family therapeutic interventions, wellness, recovery and resilience activities, and skill building as outlined in the Treatment PlanImplement Health Connect America and DMH approved therapeutic interventions as delineated on the treatment plan with clients and their families.Complete all client documentation within established
time frames
.Attend and actively
participate
in weekly Treatment Team meetings and monthly family team meetings if Wraparound Services are involved.Link the client and family with specific services and resources as
identified
in the treatment plan.Serve as a liaison with other professionals and
agencies as
related to the client.Complete monthly reviews of the treatment plan if Wraparound Services are not involved.Provide 24/7 on-call support for clients as per program requirements.Ensure compliance with all state regulatory bodies & COA standards.Participate in and complete all assigned training and professional development activities, including E Learning courses.Participate in the PQI process and strategic planning process.Employee
will include sensitivity to the service population's cultural and socioeconomic characteristics in providing treatment services.Employee must Independently transport self in his/her own vehicle to provide services in the in- home/community-based service milieu, attend meetings;
and, if
absolutely necessary
, transport clients occasionally.
Employee
must agree & accept this job duty requires him/her to
maintain
a current driver’s license, a safe driving record, current vehicle insurance; and
maintain
his/her vehicle in safe operating condition at all times
.Perform other duties as assigned to support operations and mission of the Company.
Master’s Degree in Social Work, Counseling, or a related field is required.Minimum of two years’ experience working with children and/or families, which may include internships.Must be eligible for licensure as a LPC, LMFT, or LCSW.
