Position Summary:The Lead Care Manager I is a part of a multidisciplinary team and is responsible for coordination of all aspects of Enhanced Care Management (ECM). The Lead Care Manager oversees the provision of ECM services and implementation of the care plan, conducts outreach to assigned ECM members, engages them in enrolling into ECM, and delivers ECM services. Enhanced Care Management services are provided where the member lives, seeks care, in the office, and in the community.
Qualifications:1. Lead Care Manager I - Highschool diploma or GED required.2. Lead Care Manager II - Bachelor's degree required.3. Associate of Science/Medical Assistant Certificate or Degree preferred.4. Knowledge of community resources preferred.5. Bilingual required.6. Must be 21 years old or over.7. A valid California driver's license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency's insurance company.
Essential Job Functions:1. Ability to communicate ideas in oral and written form.2. Ability to instruct in group and individual settings.3. Physical, mental, and emotional health adequate to perform duties.4. Must possess knowledge of mental health programs and community resources.5. Adhere to all applicable legal and ethical standards.6. Ability to work effectively with cultural/ethnic diversity.7. Ability to work at various locations, including client homes, schools, and community settings.8. Operate a vehicle for company-related activities, including attend offsite meetings, client home visits and transport to and from other agency locations.
Responsibilities:1. Engage and enroll eligible members in ECM.2. Oversees provision of ECM services and implementation of the care plan.3. Utilize motivational interviewing, trauma-informed care and harm-reduction strategies.4. Responsible for coordination of services to ensure a seamless experience and unduplicated services.5. Advocate on behalf of members with health care professionals.6. Connect members to other social services and support including transportation.7. Provide Community Outreach and attend Community Events.8. Provide health promotion and self-management training.9. Accompany members to office visits, as needed.10. Monitor treatment adherence.11. Coordinate with hospital staff on discharge plans.12. Responsible for preventing and controlling infection.\13. Responsible for maintaining a culture of quality and safety.14. Effectively use de-escalation skills and techniques for crisis prevention.15. Properly administer the C-SSRS to help prevent suicide.
