.Responsibilities
The
Advanced Practice Registered Nurse (APRN) - Psychiatric
is an essential team member responsible for providing and ensuring the highest standards of safety, and continuity of care. Responsible for direct services with children, adolescents, or young adults and their families including but not limited to evaluation/assessment; medication therapies; psychotherapy and counseling; parent/client education; record keeping; collaboration with other professionals, team members as well as related community services.Essential Duties:Functions as an effective member of the mental health team; oversees Psychiatric care of assigned clients and manages a regular caseload.Evaluates and manages assigned clients through direct care encounters weekly, and as needed. Direct care encounters may include prescription and management of medication, order and interpretation of labs, order of consultations or referrals, and education to clients and families.Ensure weekly follow-up appointments are performed and documented timely (within 24 hours of appointment).Timely completion of documentation including assessments, treatment plans, progress and group notes, discharge summary, according to established standards; provide utilization reviews for clients.Ensures that a Psychiatric Evaluation is performed and documented timely.Provides continuity of care by connecting to other providers upon admission to obtain history and collateral information.Attends and participates in required treatment teams, staff meetings, and training.Shares in the on-call duty responsibilities (Residential programs only).Completes discharge recommendation assessments, recommendation for aftercare, and connection with provider who will follow up with client upon discharge, as part of collaborative discharge summaries.Assists with providing cross-coverage to Psychiatry team when needed.Maintains excellent communications with other members of the clinical team, outpatient providers, and family members.If required by state law, performs services within a supervisory or collaborative environment, including maintaining requirements of Physician Supervision Agreements or Physician Collaboration Agreements.Performs other duties as assigned.Qualifications
