Oncology Nurse Navigator

to join their team!Salary/Hourly Rate:$39.90/hrPosition Overview:The Oncology Nurse Navigator will provide triage, support, and education to members during their cancer journey via phone, email, and video.Responsibilities of the Oncology Nurse Navigator:

The Oncology Nurse Navigator will establish trusting relationships with members and their care network.

Support members throughout their cancer care journey, including screening, survivorship, and end-of-life care.

Assist members with care coordination, symptom management, nutritional support, discharge planning, and provider referrals.

Assist with urgent clinical escalations and provide clinical consultative support.

Qualifications for the Oncology Nurse Navigator:

The Oncology Nurse Navigator will possess OCN and CCM.

Must possess a single-state license in Illinois.

Must have recent oncology navigation or oncology case management experience.

Minimum of 5 years of nursing experience, including 3 years in solid tumor oncology nursing.

Education Requirements:

Bachelor of Science in Nursing is required.
