Are you ready to make a meaningful impact in the lives of patients? Norwood Towers Post Acute is looking for a skilled Occupational Therapist to join our dedicated team on a full-time basis. In this rewarding role, you'll have the opportunity to provide essential care and support to patients on their journey to recovery.
Key Responsibilities:Timely Evaluations:
Evaluate patients within 24 hours (or 1 business day) of physician referral. If unable to complete the evaluation in this timeframe, document the reason in the medical record.Develop Treatment Plans:
Create effective treatment plans and obtain necessary approvals from referring physicians.Patient Care:
Treat patients according to the physician's treatment plan, ensuring the highest quality of care.Staff Training:
Assist the nursing department in training Restorative Aides and participate in in-service training programs for facility staff.Supervision:
Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in direct patient care activities, adhering to state practice regulations.Communication:
Maintain clear communication with supervisors and healthcare team members regarding patient progress, problems, and plans.Meetings Participation:
Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.Documentation:
Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation in compliance with the Occupational Therapy Board State Practice Act and payer requirements.Caregiver Training:
Instruct patients' families and nursing staff on maintenance programs and provide caregiver training in preparation for discharge.Discharge Planning:
Actively participate in discharge planning to ensure smooth transitions for patients.Equipment Management:
Recommend and facilitate the ordering of necessary durable medical equipment (e.g., braces, walkers, canes) to promote patient independence.Equipment Maintenance:
Report any issues with departmental equipment to ensure it remains in good working order.Qualifications:
Education:
Bachelor's degree in Occupational Therapy; preference for graduates of Master's or Doctorate programs.Licensure:
Must be a licensed Occupational Therapist in the state, maintaining good standing and meeting continuing education requirements.Technical Skills:
Proficient in using computer programs like Casamba and Point Click Care.Communication Skills:
Ability to read technical procedures and policy manuals; effectively present information and respond to questions from managers, coworkers, and families.Mathematical Skills:
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.Reasoning Ability:
Capable of solving practical problems and interpreting a variety of instructions in different formats.Physical Demands:
This role involves frequent standing, walking, reaching, pushing/pulling, and occasional sitting. Candidates must be able to exert force as necessary and perform tasks such as climbing, balancing, kneeling, and adjusting focus for various visual tasks.
Work Environment:
The work environment is typically low to moderate in noise level. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
