Position Overview and General Responsibilities:
Sunnyside's Therapy department has recently transitioned from a contracted team to in-house. We are currently seeking PTs/PTAs to help round out our team. As a vital team member, you will have the opportunity to support residents in meeting their therapy goals and improving their quality of life. The primary responsibilities of our Physical Therapists are to assess, plan, and administer rehabilitative programs that improve mobility, relieve pain, and increase strength while utilizing innovation and best practices to meet and exceed the needs of the residents.
Physical Therapist (Full Time/PRN):Restores patient's function, alleviates pain and prevents disabilities by planning and administering medically prescribed physical therapy.Provides quality care by assessing and interpreting evaluations and test results.Develops a treatment plan for each patient based on initial and ongoing evaluations, including short- and long-term goals, collaboration demands of other staff members, and required patient and caregiver involvement.Determines physical therapy treatment plans in consultation with physicians or by prescription.Implements Plan of Treatment on a consistent schedule and adjusts this schedule and plan according to patient progress.Helps patient accomplish treatment plan by administering manual exercises, massage, and using equipment as necessary.Evaluates mobility, strength, and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.Evaluate and record patient progress, modify treatment plans, and try new treatments if required.Evaluate the use and fit of prosthetic and orthotic devices by observing, noting patient's progress, and recommending adjustments and modifications.Documents patient care services according to approved medical documentation and facility documentation procedures.Reports patient progress to the patient, patient caregivers and loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care of patients.Develops a plan for patients following completion of their Plan of Treatment with appropriate in-home care services.Leads by example and exhibits professionalism at all times.Holds themselves accountable and encourage collaboration and flexibility within the department and interdepartmental teamwork for positive outcomes.Physical Therapy Assistants (PRN/Part Time):
Provides services under the direction of a licensed physical therapist.Contributes to a physical therapist's effectiveness by identifying patient care issues.Administers treatment programs for patientsDocuments patient care services according to approved medical documentation and facility documentation procedures.Performs therapeutic procedures by administering manual exercises and instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional movements, and daily living activities.Helps patients use supportive devices, such as crutches, canes, and prostheses.Instructs and counsels patients by describing therapeutic regimens, giving normal growth and development information, and promoting wellness and health maintenance.Leads by example and exhibits professionalism at all times.Holds themselves accountable and encourage collaboration and flexibility within the department and interdepartmental teamwork for positive outcomes.About Us:

Must hold current PT or PTA Virginia State Licensure2-3 years experience preferred
Physical and Environmental Considerations:
Normal lighting levels, temperature ranges, air quality, ventilation and noise levels. Physical stamina to lift, transfer and assist patients through physical activities. Requires frequent lifting and carrying items weighing up to 50lbs. unassisted, including assisting patients when needed. Must be able to communicate clearly to patients in person and over the phone. Visual acuity adequate to perform job duties, including visual examination of patient and reading information from printed sources and computer screens. Contact with blood, body fluids. Minimal hazards if required Universal Precautions are followed.
