Great news! We are seeking to add an Admission Coordinator to our team!
An Admission Coordinatormust proudly share our patient care vision with prospective residents, families, and community partners. The Admissions Coordinator will manage the facility's inquiry and admission process and provide customers with facility-related information.
Responsibilities and PurposeDirectly responsible and accountable for completing all admission processes, forms, and facility notification documentation.Coordinate and communicate regularly with the Post Acute Liaison and Central Intake Team to ensure a smooth care transition and admission experience.Manage facility inquiry/referral tracking reports. Serve as primary contact for all telephone admission inquiries and requests for pre-admission facility tours.Collaborate with facility leadership on customer service programs focusing on positive senior healthcare outcomes.Desired Experience and Education
Basic computer skills, including Microsoft Office.Knowledge of senior healthcare services, specifically Skilled Nursing and Long Term Care.Experienced in customer service and developing relationships within the senior healthcare environment.Working knowledge of Medicaid, Medicare and third party managed care payer sources.Business Development/Networking experience a plus.Benefits
