How You'll Make a Difference:

Arkos Health is seeking a Quality Coordinator. The Quality Coordinator will be responsible for completing in-home visits to help meet patients' needs. Visit focuses involve point-of-care testing including diabetic A1c collection, blood pressure checks, colorectal screening kits, and other testing required by HEDIS/Stars measures. This role is essential to our organization's goal of improving the overall experience of patient care, including the quality of health, and work to reduce the cost of care. For this field-based position, individuals must be able to travel up to 80% of the time in their own personal vehicle. This person will be traveling to patients' homes to ensure their quality of care.

Essential Duties and Responsibilities:

This position is responsible for performing in-home visits with patientsReview data collected to confirm gaps in care are capturedBuild positive, professional relationships with the patients we serveExplain point-of-care testing and reasoning to patientsDemonstrate excellent communication and collaboration skillsThis is both an in-office/field position: candidate must be comfortable traveling up to 80% of the timeQualifications and Skills:

High school diploma or GED equivalent requiredMust have their own reliable vehicle to drive out in the field to patients homesMedical Assistant (CMA) Certification and or Certified Nurse Assistant (CNA) CertificationCurrent CPR required2+ years' experience in a healthcare environmentAbility to perform point-of-care testing such as urine sample collection, A1c checks, blood pressure checksFamiliar with medical terminologyBenefits:
