PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs - such as food, housing, employment, health insurance, and sexual health services - across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.
New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.
This is a grant-funded position ending March 31, 2027.
Position Summary:
PHS seeks to be a data-driven organization, able to inform our activities and priorities based on what we are learning about the communities we serve and the impact of our services on those communities. The Quality and Evaluation Assistants' role will be primarily to support reporting efforts, and assist with implementation of quality improvement projects and short-term evaluations to ensure that Social Care Network services are being provided in high-need areas and to priority community groups.
We seek an experienced
Quality and Evaluation Assistant , who will report to the Senior Director, Quality and Evaluation.
The ideal candidate will:Be detail- and solution-oriented.Have a keen interest in optimizing health and opportunities in communities with inequitable health and social care outcomes.Be comfortable presenting to and engaging with various stakeholders (i.e., both written and verbal).Be an effective communicator that can adapt to a variety of audiences and environments.Be comfortable supporting continuous quality improvement projects through planning, implementation, and monitoring.Specifically, the Quality and Evaluation Assistant will:
Support WholeYouNYC Social Care Networks' evaluation efforts particularly with execution and reporting.Collaborate with teams in ongoing implementation and community-engagement efforts.Support development, administration, and data entry from surveys, as needed.Develop internal and external-facing products (e.g., health briefs, reports, journal articles) for dissemination.Generate monthly and quarterly status reports for contracted partners and other stakeholders to keep abreast of quality assurance and quality improvement efforts.Support the implementation of performance management, quality improvement and evaluation plans.Participate in team and stakeholder meetings, workgroups, and coalitions, as needed.Other duties as assigned by Senior Director of Quality and Evaluation, and in support of the WholeYouNYC Social Care Networks.Qualifications and Experience:
Bachelor's degree preferred or equivalent.1-2 years' experience with research, program planning and evaluation, and/or strategic community engagement.Experience with report writing and developing products for disseminating information.Familiarity with quality improvement methodologies.Strong understanding of health literacy principles and community- and person-centered language.Salary: $65,000 - $65,000.Desired Skills:
Adept at creating effective and engaging presentations.Strong communication (i.e., writing and verbal) and presentation skills.Some research background particularly with quantitative and qualitative data collection and summarization.Benefits:
