Valley Oasis Coordinated Entry System provides comprehensive services designed to address the root cause of poverty and homelessness within SPA 1 of Los Angeles County. Participants may be mentally ill, have intellectual developmental disabilities, substance abuse issues, and chronic medical/physical disabilities, exiting institutions and are homeless.

Under the direction of the Adults Regional Coordination Manager, and in collaboration with the Regional Outreach Coordination Team, Associate Outreach Coordinator will assist with the implementation and expansion of the Coordinated Entry System’s Outreach engagement.

Essential Job Functions:Support with coordinating outreach activities within SPA 1, encouraging team participation and fostering integration among outreach efforts.Promote effective communication among outreach teams and facilitate linkages with street-based system partners, including law enforcement, fire departments, city officials, and community-based organizations.Support the creation and implementation of strategic outreach plans in collaboration with the E6 team (LAHSA, DHS, and DMH).Support with the aid in coordination of services between regional teams, Access Centers and Access Points.Support the dispatch of memos and the scheduling of meetings to coordinate outreach efforts before and after severe weather occurrences.Support effective coordination of services and referrals to the Coordinated Entry System for Adults, Families and TAY (Transitional At-Risk Youth) populations.Support the timely management and response to LA-HOP outreach requests.Support all outreach teams use of LA-HOP and monitor response times.Promote education and technical support to other SPA 1 providers utilizing LA-HOP.Use and analyze LA-HOP data to inform necessary improvement in the outreach response system specific for SPA 1.Support with the coordination and development of CES Outreach events.Collaborate with Regional Outreach Coordinator to plan, coordinate, and facilitate CES SPA 1 outreach coordination and care coordination meetings with CES partner agencies, as requested by funder.Assist in collecting and analyzing data from outreach activities and LA-HOP to identify trends, areas for improvement, and resource allocation needs; prepare and present these findings in clear, actionable reports for stakeholders.Assist in organizing and facilitating regular meetings with outreach teams and community partners, ensuring that agendas are well-structured, discussions are productive, and outcomes are documented and followed up on effectively.Assist in developing and maintaining a centralized system for tracking outreach coordination efforts, meeting notes, action items, and progress on strategic goals, ensuring all stakeholders have access to up-to-date information and resources.Assist in setting outreach plan goals and review progress by all SPA-wide outreach teams.Collaborate with Regional Outreach Coordinator to provide regular and ongoing support, feedback, training, and guidance to all SPA-wide outreach teams.Assist in the coordination of CES SPA 1 Outreach Leadership convening, when needed.Collaborate with Regional Outreach Coordinator to evaluate and provide reports and presentations on outreach trends, efforts, and progress when needed.Ensure timely and accurate submission of required funder reports.Assist in preparing monthly, quarterly, and annual reports for program funders and other entities, when needed.Contribute and aid in the strengthening of partnerships within the SPA and engage new partners to increase available resources.Encourage all partners to actively participate in CES functions.Play an active role in helping to educate on best outreach practices in SPA 1.Collaborate with Regional Outreach Coordinator and SPA 1 outreach teams to explore solutions to current challenges, barriers, and effective services.Attend and participate in CES case conferencing meetings to coordinate services with other providers.Participate in staff meetings, individual and group supervision, case conferences and training as required.Assist in representing the Agency at various meetings held in the community; be an active participant in the community; act as a link to the community and the Agency.Research best practice models for Coordinated Entry Systems and related processes and practices and developing recommendations on how to implement them.Participate in efforts to identify and obtain community resources to support the program and to raise awareness of homelessness prevention.Resolve grievances in accordance to CES grievance procedures.Adhere to agency policies and procedures including confidentiality, child/elder abuse reporting, and code of ethics.Additional tasks, projects and or responsibilities as assigned by Supervisor that are not outlined but which are an important part of the agency’s goals and/or program outcomes.Knowledge, Skills & Abilities:Clear understanding of the concepts of Housing First, Harm Reduction, Trauma Informed Care, Motivational Interviewing, and the Coordinated Entry System.Strong knowledge of case management ethics, HIPAA, Programmatic standards of operation and participant focused ideology.Knowledge and understanding of policy, systems, and local programs that impact homeless persons and related public funding sources, regulations, requirements, and procedures.Knowledge of best practice models for assisting homeless people with varying degrees of needs/conditions and have a strong understanding of contractual requirements/compliance and applicable performance measures.Knowledge of issues facing homeless individuals (e.g. chronic health, substance abuse, mental health, domestic abuse and resources for undocumented individuals).Knowledge of community social service agencies and resources.Ability to respond quickly and effectively to individuals who may be ill, using substances, lack of personal care, in crisis situations and/or potentially hostile situations.Ability to work with a diverse population and provide culturally competent advocacy.Actively promote positive, nurturing interactions between participants.Work effectively under conditions of limited supervision, high stress and rapidly changing situations and circumstances.Able to take appropriate steps to deal with emergency situations with the goal of maintaining the safety of all participants.Promote cooperation and commitment within a team to achieve goals and objective; collaborate with team members, sharing ideas and differences openly; be receptive to new ideas and adapt to change as necessary.Strict confidentiality must be observed as it relates to client information, agency business, program decisions and all other personal information.Considers impact of personal and professional choices. Consistently make decisions in keeping with organizational values and Supervisor’s direction.Excellent organizational skills and the ability to follow through from beginning to end on tasks.Outstanding communication skills, both written and oral; must communicate professionally and positively with participants, co-workers, management, community partners, employers, landlords, and other service providers.Proficient with office communication equipment and machines (telephone, copier, calculator, fax machine, personal computer, laptop, computer printer and scanner).Proficient in use of laptop/desktop computer, Microsoft Windows and Office software programs (Word, Excel, and PowerPoint) and Google drive.Ability to work a flexible schedule; including nights, days, weekends and holidays.Ability to respond to emergencies some evenings and weekends as necessary.Use of a personal vehicle may be required to travel in and around Antelope Valley and/or Los Angeles County and periodically transport participants.Maintain professional boundaries and work collaboratively with participants, volunteers and staff.Accountability – be on time for scheduled shifts.Be able to read and write English.Bilingual Preferred but not required.Qualifications & Experience:Bachelor’s Degree in Social/Human Services or related field, preferred.Experience working with a wide range of homeless and social services providers, City and County departments, elected officials.Two year experience in social services setting; preferably working as an employee or volunteer in providing homeless services.Strong knowledge of the social services field and/or successful management and implementation of homeless programs in the non-profit, government or private sector.Experience with Homeless Management Information System (HMIS).Education may be substituted by years of experience.Agency Requirements:Must have a valid California Driver’s License.Reliable transportation.Auto Insurance.Clean driving record.Employment pending drug screen, background check and driving record.Work Conditions & Physical Demands:The following are work conditions that one may encounter in the course of performing their job duties:May be in contact with individuals in crisis.May experience unpleasant sensory demands associated with an individual’s use of alcohol and drugs, and lack of personal care.May experience crisis situations and potentially hostile situations, must be ready to respond quickly and effectively.An employee is required to:Walk and climb stairs; Handle, grasp and feel objects and equipment; Reach with hands and arms; extend both arms above the head and/or reach below the waist; Communicate ideas and information verbally and written word; Be able to move oneself quickly and easily from place to place; Repeat various motions with hands, wrists, fingers; Climb on stools, steps and/or ladders; Be subject to outside environmental conditions, including but not limited to: Rain, heat, wind; lift at least 10 lbs.
