This position is a grant-funded position for 3 years, with opportunity for renewal and increased FTE as the program expands.We are currently seeking a Clinical Coordinator to join our Ryan White Positive Care Clinic team (and Addiction Medicine Clinic team). This part-time role will primarily work day shift on-site on the Minneapolis downtown hospital campus. This is a new grant funded position from a Hennepin County contract that was executed on March 18th, 2024, and set to expire December 31st, 2025. The grant will support .06 FTE of a nurse coordinator position to work collaboratively to support a syringe exchange in both Positive Care and Addiction Medicine, with opportunity for increased FTE as the program expandsSUMMARY:The Clinical Coordinator is responsible for facilitating and coordinating the care delivered to an assigned group of patients through multidisciplinary collaboration to ensure quality and cost effective outcomes are delivered ensuring appropriate use of resources in a patient-centered manner. Coordination involves assessment, planning, support, and evaluation of patient care and related outcomes. Activities to be performed are patient interviews, clinical resource management, referrals, lab coordination of care between Positive Care and Addiction medicine as well as the larger organization.The Clinical Coordinator is an active contributor in the development of systems to improve the care coordination of assigned patient populations.RESPONSIBILITIES:Provides Care Coordination for patients, physicians and care providersPlan, implement and evaluate transition functions and activities for a caseload of patients with substance use disorders across the continuum careContribute to the development and implementation of individualized patient care plans that meet patient population goals/standardsEnsure implementation of the treatment plan including screening for infectious complications of drug use, referrals to substance use treatment facilities, infectious diseases, or community partners, and coordination of phlebotomy for screening labs to achieve clinical outcomes consistent with the needs and preferences of the patient
Provide services to streamline plans of care, reduce hospital readmissions, ease the transition to long term services and supports, and interrupt patterns of frequent hospital emergency department useFacilitate implementation of best practice standardsMaintain plan of care across continuum of careResponsible for a caseload of patientsActively track outcomes and participate in quality activities specific to patient population/departments servedBe responsible for organization and documentation and tracking data and outcomes relevant to grant activities both in EPIC and in the kit distribution log in both clinical departmentsAssembly of harm reduction kitsProvide information/education to healthcare providers to support changes for care delivery to patients and patient populations that meet quality, cost, and service goalsDelegate tasks as appropriate to other members of the health care teamPromote effective working relations and works effectively as part of a department/unit team inter and intra-departmentally to facilitate the department's/unit's ability to meet its goals and objectivesDemonstrate respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment; approach all healthcare encounters in a nonjudgmental, open, destigmatized, patient-centered care mannerDemonstrate understanding of diversity issues related to patient careUnderstand and meet the requirements of the Corporate Compliance Program; complies with the standards set by department policy, The Joint Commission and other regulatory agencies governing activities within the department 100% of the time
QUALIFICATIONS:
Minimum QualificationsBachelor's degree: BSN, BAN
HCMC employees who became Clinical Coordinators (Sr. Staff Nurses) prior to August 13, 2012 and do not possess a Bachelor's degree in Nursing are considered qualified.Must have worked as a professional nurse for the past three to five yearsMust have experience in acute care and/ or ambulatory careMust have demonstrated professional leadership (i.e., charge, team leader, preceptor, committee chair, etc.)Current Minnesota RN licensure
Knowledge / Skills / AbilitiesFor any unit based specific posting, previous experience in that specialtyPreferred QualificationsCase management certification preferred but not required
One year of experience at Hennepin County Medical Center strongly preferred
Professional certification in specialty area of clinical expertise
Involved in a professional nursing organization
Phlebotomy experience, particularly with difficult draws, preferably ~ 5 years or more
Harm Reduction Program experience, particularly with experience in starting a new program / service
Experience working with individuals living with HIV and/or SUD
