Job SummaryProvides specialized nursing care within the LPN scope of practice to patients under the direct and indirect supervision of the Congestive Heart Failure/Telehealth RN Program Coordinator, RN Case Manager, and Nurse Manager. Responsible for communicating with other disciplines and agencies involved in patient's care.
Essential Functions

Installs telemonitoring equipment as directed by RN Home Health Program Coordinator or designee.
Electronically monitors telehealth transmissions under the direct and indirect supervision of the RN Home Health Program Coordinator or designee.
Maintains a proactive approach in assisting patients and families, including trouble-shooting telehealth equipment, under the supervision of the Registered Nurse.
Reports changes in patient condition to RN Home Health Program Coordinator or RN Case Manager and initiates appropriate interventions to meet patient needs when there is a change in the patient's condition.
Provides safe and quality healthcare services and documents such per agency policy.
Demonstrates competence in the use of equipment and maintains knowledge through continuing education.
Retrieves telehealth equipment from patient's home, cleans equipment, and returns to inventory.
Maintains knowledge of medications.
Educates staff, patients, and caregivers in the use of telemonitoring equipment.
Performs other duties as assigned.
Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.
Minimum RequirementsEducation - Vocational / Job-related special education - LPN
Experience - Two (2) years Nursing experience

Required Certifications, Registrations, LicensesLPN
If an LPN team member working in this position obtains RN licensure, Prisma Health will accept RN licensure for a limited time until the team member can be placed in an RN position.
Team members should immediately notify their manager and Talent Acquisition if they are scheduled to sit for the NCLEX-RN exam or have obtained RN licensure.
