The Program Specialist is responsible for upholding Abound Health PA, LLC's mission, vision and values by providing support, services, education and compassion to Clients receiving Pennsylvania Waiver or Base Funded Services.

Review assigned client's service/grid notes within a timely mannerReview all verification/documentation of services for appropriate signatures and documentation per the ODP Records Management and Documentation ManualEnsure assigned direct care staff records are currentEnsure assigned client clinical records are currentConduct and document clinical and employee supervisory visits to ensure Abound Health PA's Policies/Procedures are followedConduct annual performance evaluations, direct care staff development plans, disciplinary meetings/documentation with direct care staffParticipate in required staff meetingsParticipate and advocate for the client in the client's ISP meetingsInform plan members of the option to decline the ISP Review documentation as requiredDevelop individualized short-term goals, task analysis and interventions for assigned clientsComplete client assessments (as applicable for 2380, 6400/6500, agency required, etc.)Ensure client specific trainings are completed for every client/staff relationshipEnter goals into OnTargetReview weekly billing to ensure hours are usedComplete monthly/ quarterly progress reportsRecommend a revision to a service or outcome in the ISP as neededReview and update task analysis per client's goal frequencyEnsure staffing patterns are covered and back up staff is availableResolve and document client/guardian grievances and report to SupervisorComplete significant event and incident reports within 24 hours (client and employee/contractor)Schedule direct care staff to provide direct care. Hours must comply with service authorizations and waiver limitationsAssist the clients and families with finding and procuring appropriate resources and services in the communityRepresent Abound Health PA at Community EventsPerform daily activities per company's needsKnow and implement regulations associated with 2380 and 6400/6500 service programmingGENERAL RESPONSIBILITIES

Provide privacy and confidentiality of all client records.Assure clients are free from abuse, mistreatment, and/or neglect and report any incidents of such to supervisor.Treat each client with dignity and respect.Develop and demonstrate a thorough understanding of Abound Health PA's philosophy and purpose.Provide a positive atmosphere which facilitates growth and learning of coping skills.Perform duties as requested by SupervisorSKILLS/EXPERIENCE/QUALIFICATIONS

Must be credentialed as a Program Specialist with either:

Graduate of a college/university with a Master's degree from an accredited college or university and 1 year of work experience working directly with persons with disabilities.Graduate of a college/university with a Bachelor's degree from an accredited college or university and 2 years of work experience working directly with persons with disabilities.Graduate of a college/university with an Associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with persons with disabilities.Requirements

Program Specialist QualifiedAt least 18 years of ageValid Driver LicenseSafe Driving RecordNo substantiated findings of abuse and/or neglectCriminal, PA State Police, FBI (if required), SAM, MEDI Check, Sex Offender, LEIE, PA Child Abuse History (if required), Driving Record checks clear of health and safety risks.Reliable and safe transportationCurrent Auto Insurance, Vehicle registration and inspection (if in applicable state)

Work Environment

This job operates in the Abound Health PA Administrative Offices, Client's home and various community settings (library, parks, shopping areas, Abound Health PA Day Programs, etc.).

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this position, the employee is frequently required to remain in a stationary position, move about inside and outside an office setting, and operate a computer and other office machinery; detect and communicate accurate information often via multiple media types. The employee must occasionally transport or move up to 25 pounds.

Travel

Travel is expected from Abound Health PA Administrative offices, Direct Care Staff homes, Client homes, and community events/activities within the assigned region.
