JOB PURPOSE
:
The Community Health Coordinator (CHC) serves as a business development liaison, consultant, and patient advocate partner with Ventanilla De Salud (VDS), Health Builders (HB) of El Rio Community Health Center, and Grant Funders such as the City of Tucson Lead-Based Paint Hazard Control Grant (LBPHCG). As the designated liaison, the CHC collaborates with local community centric and health care agencies, educational institutions, and clinical team members to facilitate unmet heath care needs of El Rio Community Health Center patients and their families. The Community Health Coordinator will participate and collaborate in the research, development, and creation of a program's educational and marketing materials, program lectures, and community events, which used for distribution to program clients (internal/external), community members, and associated outside agencies and organizations.
The primary goal of the El Rio Community Health Center Community Health Coordinator is to support El Rio's Mission of providing comprehensive, quality health care that is affordable and accessible to all who may have healthcare needs, by successfully performing the primary essential functions.

or additional Grant Projects assigned to the HB Team or Ventanilla to ensure that all goals and objectives specified for the program are successfully accomplished in accordance with the established priorities, timelines, assigned budget, and deliverables.Coordinates, consults on, and oversees health promotion and health education activities for individuals and groups including; outreach activities in the community, group visits in the clinical setting, receives and consults on referrals from clinical team members for addressing basic needs for advocacy, reinforces needed education and self-management skills, coordinates care management with other clinical team members.Researches, plans, and coordinates activities of the program with interrelated activities of other programs, departments, agencies, or organizations ensuring optimum efficiency and productivity of the program goals and objectives.Develops written or visual multimedia materials for use with all program events, lectures, or training sessions for presentation to program staff, internal/external clients, community members, or associated outside agencies and organizations.Provides monthly outreach and presentations for community agencies, churches, schools and other identified groups in need of access to health care.Supports and/or completes all assigned program projects; performs and/or completes administrative duties, tasks, and assignments supporting program operations within established time frames; meeting established rates of performance for the quality and quantity of work for the position; demonstrating a level of quality, efficiency, and accuracy in the employee's job performance that ensures the highest standards of excellence.Participates and collaborates in the research, development, and creation of program educational or communication materials for distribution to program clients, internal/external clients, community members, or associated outside agencies and organizations.Identifies and learns more about health concerns and illnesses reflective in specific clinic populations through education; therefore, able to provide basic health education for patients and families. (i.e., diabetes, seniors, high risk pregnancy, immunization, etc.)Teaches culturally appropriate, effective health promotion and disease prevention classes and workshops for target population at specific clinic locations.Contacts targeted participants and providers to assure class and/or group visit attendance, adherence to self-management activities, and follow up with providers.Facilitates training sessions, events, or meetings sponsored by the program, ensuring the successful presentation of information or services to all program clients and event participants.Coordinates and assists with planning and logistics for community events and events at specific clinic locations.Intake of clients referred to Lead Testing by the City of Tucson (LBPHCG); adding referrals as clients and scheduling appointments.Maintains and provides reports to Grantors (City of Tucson or other Grantors); quarterly lab reports, lab results, invoicing, workflow, and attends and presents information at City of Tucson monthly meetings.Coordinates and supports the evaluation and monitoring of program activities and/or grant-funded programs, and assists in the writing of specifications for evaluation or monitoring of the program(s).Gains and maintains a comprehensive understanding and proficient ability to ensure full compliance of the program with all regulations, standards, or requirements for all Federal, State, and local laws; as well as all standards and requirements for internal/external grants or funding sources.Supports and participates in the recruitment, orientation, and training of staff and/or volunteers to the various required duties, responsibilities, and assignments within the programAssists and supports in the analysis and prioritization of staff and/or volunteer work assignments that will support successful achievement of program goals and objectives.Create and keep current a monthly calendar of health center programs, activities and outreach events specific to assigned community sites and organizations.Other Duties and Responsibilities:
Maintains at all times employee and/or patient confidentiality by controlling the environment and information being disclosed to authorize individuals ensuring compliance with all HIPAA and corporate compliance standards, as well as accepted human resources confidentiality standards.As required, may coordinate, monitor or supervise program staff or volunteers providing guidance and direction on the successful completion of assigned tasks, duties, and responsibilitiesDemonstrates an exceptional level of customer service; answer and respond to all incoming calls, emails, and inquiries in a timely and effective manner, responds to requests for support providing general information in response to inquiries; referring technical inquiries or complaints to the program member for resolution.Takes a leadership role in representing El Rio and the HB Team in community organizations and events.Maintains relationships with community resources and agencies that support the El Rio and HB missions.Demonstrates an understanding of and proficiency with the application of all Joint Commission Accreditation standards and reporting requirements applicable to a Program Coordinator.Supports and participates in the recruitment, orientation, and training of staff and/or volunteers of their required duties, responsibilities, and assignments within the programAssists and supports in the analysis and prioritization of staff and/or volunteer work assignments that will support successful achievement of program goals and objectives.Maintain a current diverse list of community agencies and resources for patients and their families to maintain and improve their health statusFollows up with all patients regarding desired education, group visits or other health care needs within 48 hours of query or referral.Accurately documents all patient services in the EMR and NextGen as needed for clinical excellence.Maintain data related to all programs provided, coordinated and offered for patients and the community including but not limited to the number of participants, those needing follow up, screening tools for measure with an emphasis on outcomes, etc.
Gains and maintains a comprehensive and advanced knowledge, skills, and expertise on governmental and private healthcare programs, such as:Affordable Care Act, Healthy Arizona Plus, Sliding Fee Scale
Supports and assists consumers by providing explanations, enrollment assistance, and information on eligibility options and requirements for programs such as: Medicaid (AHCCCS), ACA Marketplace, and discount programs (PCAPParticipates and assists in the development, preparation, and reconciliation of the program budget(s).Gain and maintain a comprehensive understanding of all compliance, performance, and reporting requirements or standards required for grant participation.This job description has been reviewed to ensure that the minimum essential functions and basic duties have been included to successfully perform the duties, tasks, assignments, and responsibilities of this position.
