SUMMARY: Licensed Clinical Social Worker (LCSW) serves as a part of a multidisciplinary team will provide direct services including individual (including youth), couple, family and group therapeutic services to patients, including assessments, treatment plans and discharge planning. LCSW advocates and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. LCSW is expected to behave ethically and professionally, demonstrate excellent communication skills and be capable to work with diverse populations. Since they interact with professionals from many different social service and government agencies, they must be able to collaborate well with others. The profession also requires emotional resilience and stability since the work often involves dealing with crises.

QUALIFICATIONS/POSITION REQUIREMENTS: These specifications are general guidelines based on the minimum experience normally considered essential to the satisfactory performance of this job. Individual abilities may result in some deviation from these guidelines. Additional requirements include: 1) knowledge of electronic medical records; 2) knowledge of community and community resources; 3) ability to pass background check; 4) valid North Carolina State Driver's License; 5) TB Clearance; 6) flexibility to work overtime, holidays and weekends; 7) local travel required.
EXPERIENCE / KNOWLEDGE: To perform effectively in this position, the incumbent must have/be:
EDUCATION: Master's Degree in Social Work or related field from accredited university. LCSW in good standing in the State of North Carolina with minimum two years post-licensure.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Addresses referrals from providers including referrals via warm hand-offs; Consultation with physicians, medical professionals and therapists when appropriate
- Participates and daily huddles to assess needs and coordinate services
- Maintains weekly scheduled appointments in our electronic records systems called Greenway Intergy
- Provide therapeutic services to patients and their families
- Work in collaboration with multidisciplinary team members including external partners
- Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner
- Demonstrates cultural competency and commitment to patients
- Demonstrates commitment to the philosophy of the corporation
- Maintain case load of 15 to 20 patients weekly, and minimum of 5 patients a day
- Providing emotional and mental health counseling to groups or individuals
- Building strong rapport with clients and making accurate assessments of their problems whether mental/emotional or physical in the case of substance abuse
- Keeping detailed and concise records of each patient during the course of their treatment
- Performing research tasks within a clinical setting
- Other duties as assigned

- Working knowledge of Clinic policies and procedures
- Licensed Clinical Social Worker (LCSW) from the North Carolina Board of Behavioral Sciences.
- Knowledge and experience with evidence-based practices, including trauma informed care framework.
- Knowledge and experience working with co-occurring populations (chronic health conditions, mental health and substance use) with emphasis on children and adolescents.
- Demonstrated oral/written skills. Strong clerical/administrative skills including personal computer skills (Microsoft Office software).
- Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures.
- Ability to work effectively with people of diverse cultures, ages and economic backgrounds a cultural competent and sensitive manner.
