Join our Hospice team at PeaceHealth!
We are hiring a part-time Nurse Practitioner to support our Hospice patients providing direct patient care, joining a team of 3 physicians, and 1 Nurse Practitioner.
+ Busy practice (200 - 230 patients)
+ Face to face visits with patients for re-certification of hospice services
+ Working closely with PeaceHealth community nursing team, general inpatient and inpatient support of end-of-life care
+ On call rotation 1:4 Monday - Monday
+ Collaborative team environment

**Job Summary**
Within scope of practice and in alignment with both hospital staff bylaws and PHMGs physician supervision expectations, responsible for advanced level of nursing evaluation, clinical assessment, prescribing and monitoring medications and follow-up care for patient. Responsible for patient and family teaching.
