**Description:**

Eddy VNRA has a unique opportunity to work full time as part of our Liaison Team!

If you are looking for a fast paced position this is for you!

Primary responsibilities include:

+ Communicating with the hospital case managers, Rehab and hospital services, community physicians, and various resource agencies

+ Coordinating services for patients discharging home from an acute care setting

+ Contacting physician offices, obtaining various services/orders (ie: DME, infusion) as needed for the homecare clinicians and ensuring referral information/orders are complete

+ Documenting patient information into our EMR

+ Working with various departments to coordinate a smooth transition to home

Key skills required:

+ Strong interpersonal skills

+ Excellent computer skills

+ Ability to multitask multiple patient referrals

+ Organization

+ Team player

+ Strong time management and clinical skills

Pay Range: $31.50 - $42.80

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
