Job Description
General Summary
The Home Infusion Navigator manages the care and service of patients being discharged to a home infusion setting. He/ she works collaboratively with physicians and other members of the health care team to achieve the highest quality clinical outcomes with the most efficient use of available resources. The Home Infusion Navigator assumes responsibility for interfacing directly with clinicians within referral sources to facilitate a home infusion referral. This may include providing assistance with discharge planning when appropriate.
Principal Responsibilities and Tasks

* Conducts assessments of patients selected for possible home infusion care. Collects and reviews pertinent patient data to assist the Intake Department.* Reviews the patient's record to obtain primary and secondary insurance information to assist the Intake Department.* Effectively communicates with agency staff, medical team, patients and family throughout the discharge process. Ensures seamless transition to home.* Educates patients and their families about the services that will be provided to patients needing home infusion therapies.* If license permits, conduct bedside patient hookups for specific therapies prior to discharge to home.* Coordinates and communicates all critical workflows activities for new referrals to the following team members when appropriate - Intake, Clinical Operations (Pharmacy and/or Nursing) or others.* Develops, maintains and grows core therapy and service line admission to the company.* Maintains up to date referral source records.

Qualifications
Education and Experience
* 4 year/ Bachelor's degree in Nursing or Social Work (Required)* Master's Degree in Nursing or Social Work (Preferred)* Active RN or Social Work license (Required)* 1 year of experience in a similar role in the Home Infusion business is required.* Home Health or Home Infusion preferred or minimum of two years of case management or discharge planning experience.
Knowledge, Skills and Abilities
* Strong Verbal Communications Skills, Strong Written Communications Skills, Excel - Intermediate Level, PowerPoint - Intermediate Level, MS Word - Intermediate Level, Excellent Organizational Skills sufficient to maintain accurate records.
