-Combined inpatient and clinic-based practice-3 years of inpatient bedside nursing experience preferred-Candidates should be acute care certified or must be eligible to take ACHPN examination within 6 months of employment.-Palliative or critical care experience preferred
This position requires providing service to the following age population(s): 18 and above in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
The Palliative Nurse Practitioner provides quality, patient-centered care to patients in a the inpatient and clinic settings, under the direction of collaborating physician(s), including the following:
• Managing the care of palliative inpatients/outpatients, including assessment of problems, symptom management, and coordination of medical care.
• Taking complete history and physicals, recommending diagnostic and therapeutic studies, medications, writing complex medication orders (when necessary), obtaining informed consent, and evaluating response to treatment.
• Renews prescriptions for clinic patients
• Provides palliative specific education and support to patients and caregivers.
• Assists in obtaining third party approval for specialty medications when necessary.
• Manages care of patients who are receiving ongoing, disease directed treatments or those who may not be candidates for further treatments
• Utilizes the Einstein Electronic Medical Record system in documenting and managing patient care.
• Provides holistic, quality and cost-effective advanced practice care for patients with chronic or acute, complex illness.
• Participates on committees and with resident/fellowship education.
